What are the signs of cervical insufficiency?

One is a congenital disease. The cervical wall of patients in this situation is relatively thin, the load-bearing capacity is relatively poor, or the cervical position develops relatively rapidly. In addition, most of these patients also have symptoms of uterine enlargement and small uterus. The proportion of cervical fibrous tissue, collagen fiber and smooth muscle in patients with this disease is relatively small, which may be caused by the rupture of fibrous tissue inside the cervical cavity, resulting in reduced dilation ability, and finally causing physiological expansion and relaxation of the cervix.

Second, patients with cervical erosion have undergone cervical conization surgery, which usually leaves scars on the cervix, damages the strength of the cervical muscles, and even shortens the length of the cervix.

The third is people who have experienced multiple abortions. When the cervical canal is still open for hours, some hard lumps force their way through the cervix into the uterus, causing the cervix to relax and even cause tearing.

Fourth, some female patients with polycystic ovary usually have symptoms of uterine insufficiency, and the occurrence of this condition is related to a person's growth hormone.

What to do if you have an incompetent cervix?

General medical treatment

Blood pressure bed rest;

Blood estrogen: 17-hydroxyprogesterone value 500-100b250g/week;

Supplement uterine contraction retarder, such as β-adrenergic hormone protein kinase stimulant rodopa, first intravenous injection, after uterine contraction is suppressed, change to oral administration 4b250g, once every 8 hours. Pay attention to the heartbeat of pregnant women during the whole process of taking the medicine. Those with heart disease are prohibited from using the medicine.

Weighted silicone sleeve: Place the sleeve as far as possible inside the vagina around the cervix, and allow it to gradually expand. Remove it at the end of pregnancy or when labor starts.

Surgery

Surgical methods during non-pregnancy include scraping method, Palmer-Lash method, and Mann method. Its main purpose is to strengthen the muscle wall by removing the weak parts of the cervix and isthmus of the uterus or causing local fibrosis, but it may cause secondary cervical scarring and infertility or difficulty in expelling uterine contents, so it is rarely used.

Surgery during pregnancy: The operation is usually performed between 14 and 18 weeks of pregnancy, but in recent years surgery at 20 weeks of pregnancy has also achieved good results. B-ultrasound can also be used for testing after pregnancy. If the diameter of the internal cervical os is ≥ 23mm or the fetal membrane is bird-beak-shaped and funnel-shaped, and the cervix is ​​2b250m long, surgery is performed.
