Advantages of subtotal hysterectomy

Laparoscopic hysterectomy began in 1989. With the development of various surgical instruments, this procedure has been widely carried out. Although LAVH is easy to perform because of the relatively few operations under laparoscopy, the vaginal surgery part of LAVH is very difficult for some difficult cases, such as endometriosis and pelvic adhesions, which make it difficult to pull the uterus down. For such patients, laparoscopic hysterectomy is relatively easy because it is performed entirely under laparoscopy. Compared with abdominal hysterectomy and vaginal hysterectomy, laparoscopic surgery has a clearer field of view. For patients with combined endometriosis and pelvic adhesions, laparoscopic hysterectomy avoids the difficulties of vaginal surgery and the trauma of open surgery, expanding the scope of minimally invasive surgery, and is more advantageous.

While performing laparoscopic total hysterectomy, laparoscopic surgical treatment of other diseases can also be performed, such as endometriosis lesion resection, ovarian tumor resection, vaginal stump suspension, laparoscopic pelvic floor defect repair, bladder neck pubic ligament suspension, etc. Laparoscopic lymph node dissection can also be performed to treat endometrial and cervical cancer.

Compared with open hysterectomy, it has a smaller incision, fewer postoperative complications, less chance of needing postoperative analgesia, and can allow patients to return to normal work and life more quickly. The small laparoscopic incision is more beneficial to obese patients, as it provides a clear field of vision during surgery and avoids the problem of poor healing caused by large abdominal incisions.

Selection Principles

Laparoscopic, vaginal and abdominal hysterectomy, the first two are minimally invasive and can be recovered much faster than abdominal hysterectomy, while there is no difference in postoperative recovery between laparoscopic and vaginal surgery. Despite this, laparoscopic hysterectomy still has advantages that vaginal hysterectomy cannot match, mainly that it can provide a clear understanding of the pelvic condition, has a clear surgical field, and can simultaneously treat lesions coexisting in the pelvic cavity. For complex cases, laparoscopic surgery is much safer than vaginal surgery. Therefore, for simple total hysterectomy, vaginal surgery or laparoscopic surgery can be the preferred surgical procedure. When uterine hysterectomy is required due to combined pelvic adhesions, endometriosis and other diseases, laparoscopic hysterectomy should be the preferred surgical procedure. However, both laparoscopic and vaginal hysterectomy have their limitations, namely, they cannot complete hysterectomy in patients with giant uterus or severe pelvic adhesions. For such patients, laparotomy should be chosen with sufficient estimation before surgery, or converted to laparotomy if difficulties are encountered during the operation.
