In terms of evaluating ovarian reserve function, anti-Mullerian hormone (AMH) testing can make up for the shortcomings of traditional hormone testing and quickly and effectively determine the level of ovarian reserve capacity. A woman's fertility is closely related to her ovarian reserve function and various reproductive hormones in her body. Insufficient ovarian reserve capacity can cause women to age prematurely, prolong their menstrual cycles, aggravate cardiovascular problems, and even lead to symptoms such as menopause, climacteric, and infertility. Correctly evaluating ovarian reserve function is the key to preventing female aging and infertility. AMH testing has the following clinical applications: Assessment of ovarian reserve Compared with other traditional biological indicators, AMH has many obvious advantages in assessing ovarian reserve and is the most accurate biomarker of ovarian aging [2]. AMH reflects the trend of ovarian reserve decline with age earlier than FSH, estradiol (E2), inhibin B (inhB) and antral follicle count (AFC) [11], and its level is not affected by the menstrual cycle, hormonal contraceptives and pregnancy [12]. Although studies have shown that AMH fluctuates during the menstrual cycle of women, especially young women, its fluctuation is much smaller than that of other sex hormones. Therefore, existing guidelines mostly support testing AMH at any time during the menstrual cycle. In addition, Fanchin et al. compared the repeatability of AMH and other ovarian reserve markers between different menstrual cycles. AMH is a hormone that satisfies the intra-class correlation coefficient>0.8, indicating that the variation of AMH between menstrual cycles is also small. Therefore, AMH is the most stable and reliable indicator of ovarian reserve function. Assessment of ovarian reserve in cancer patients Surgery, chemotherapy, and radiotherapy may all affect the ovarian reserve function of cancer patients. A 10-year follow-up study showed that the AMH levels of childhood cancer survivors were significantly lower than those of healthy peers, and the rate of infertility continued to increase. Among breast cancer patients, those with lower preoperative AMH levels were less likely to resume menstruation within one year after surgery. Among patients undergoing ovarian cystectomy, those who used electrocoagulation to stop bleeding had lower AMH levels than those who used sutures or hemostatic materials. In addition, evaluating serum AMH levels before chemotherapy can be used to predict ovarian function after chemotherapy. A prospective study showed that the lower the AMH level of patients with early breast cancer before treatment, the more likely they are to experience amenorrhea after chemotherapy. The speed at which AMH levels recover in cancer patients after chemotherapy is inversely proportional to the ovarian toxicity of the chemotherapy drugs. It can be seen that AMH levels can predict the ovarian function of cancer patients and help patients who wish to preserve their fertility choose medications and surgical methods. |
