Right ovarian enlargement

Right ovarian cyst refers to ovarian cyst, and right adnexal cyst refers to right ovarian cyst. Ovarian cyst is a common benign tumor of the female reproductive organ, which is called "stone mass" or "intestinal mass" in traditional Chinese medicine. It is divided into mucinous cystadenoma, serous cystadenoma, benign teratoma, fibroma, testicular blastoma and other types. Most of them have the possibility of malignant transformation and are common in women aged 20-50 years old.

Cause

Reason 1: Environmental factors. Unreasonable dietary structure, high cholesterol diet, vitamin A, C, E deficiency, smoking, ionizing radiation and other environmental factors are related to the occurrence of ovarian cysts;

Reason 2: Endocrine factors. The incidence rate is increased in nulliparous or childless women; early menarche (before 12 years old), menopause after 50 years old, frequent menstruation, use of ovulation induction agents for more than 3 cycles, precocious puberty or virilization are also related to the occurrence of ovarian cysts;

Reason 3: Bad living habits. Bad living habits, excessive psychological stress and other factors cause excessive acidification of the body, which reduces the overall function of the human body, causing kidney deficiency, which in turn causes the metabolic cycle in the lower part of the body to slow down, resulting in endocrine disorders, decreased immune function, and ultimately ovarian cysts;

Reason 4: Congenital factors. If the body cells are in acidic body fluids, the amount of oxygen dissolved in normal cells will decrease, causing the cell activity to decrease and the metabolic cycle to slow down. Normal cells will not be able to survive and the cell phenotype will change, thus forming a cyst entity.

1) It can easily lead to female infertility: Once the ovarian cyst becomes larger or degenerates, it is possible that the sperm, egg or fertilized egg cannot function normally, thus affecting fertility and leading to infertility. In addition, ovarian diseases such as congenital absence of ovaries, immature ovaries, premature ovarian failure, polycystic ovaries, and ovarian tumors can affect the ovulatory function of the ovaries, leading to infertility. Early ovarian failure and premature menopause in women can cause their ovaries to stop functioning prematurely, leading to infertility.

2) Premature aging of female appearance: Ovarian cysts can cause endocrine disorders, premature aging, malignant changes, and even ovarian cancer. 3) Cause miscarriage and dystocia: Ovarian cysts may cause miscarriage in early pregnancy, ovarian cyst pedicle torsion is prone to occur in mid-pregnancy, and larger cysts in late pregnancy may lead to abnormal fetal position, which may block the birth canal and cause dystocia during delivery. 3) Cyst rupture and life-threatening: When an ovarian cyst grows to a certain size, complications may occur: infection, cyst pedicle torsion, bleeding or rupture, which may endanger the woman's life.

Benign surgical treatment

① Ovarian cystectomy. Most of these patients have no menstrual disorders and some even have complicated pregnancy. If the tumor is more prominent on one side, a salpingo-oophorectomy on the affected side can be performed.

② Salpingo-oophorectomy. For unilateral ovarian cysts occurring in older patients (over 45 years old), unilateral or bilateral ovarian cysts are usually treated with unilateral or bilateral oophorectomy. For patients who are not fit for surgery due to general condition or who have severe inflammation, total hysterectomy is often performed. It is worth noting that for the surgical treatment of larger ovarian cysts, the size of the incision should be disregarded and complete resection is appropriate to avoid rupturing the patient's pulse and allowing the contents to spill into the abdominal cavity or incision. During the operation, attention should be paid to the patient's pulse, breathing, and blood pressure changes. When necessary, infusion or blood transfusion, oxygen infusion should be accelerated. Early detection of acute gastric dilatation, paralytic intestinal obstruction, and the resulting water and electrolyte imbalance and blood chemistry changes should also be prevented.

③Hysteroscopy and laparoscopy. Hysteroscopic and laparoscopic technology is a major breakthrough in the field of minimally invasive surgery. As an effective diagnostic method for intra-abdominal diseases, it has been fully practiced in clinical practice. It is mainly used in gynecology, such as fine needle puncture of ovarian cysts, separation of pelvic adhesions, and correction of fallopian tube obstruction, torsion, and peripheral adhesions. Adnexectomy and total hysterectomy are suitable for women who are near menopause or menopause and have unilateral or bilateral ovarian cysts. If the patient's general condition is not suitable for the surgery, bilateral adnexectomy and total hysterectomy are recommended, but this will seriously affect endocrine disorders.

Malignant surgery

Because most patients are in the late stage when they seek medical treatment, it is necessary to do everything possible to remove the primary cyst and any visible pelvic and abdominal metastases. Because malignant ovarian cysts are often adhered or infiltrated with the uterus and adnexa, and are closely attached to the pelvic peritoneum, the uterus and tumor are often removed in one piece, such as by rolling carpet or dumpling-like removal of the greater omentum, partial intestinal resection, partial bladder and ureter resection. For ovarian malignant tumors complicated with ascites, regardless of whether they are completely removed or not, it is advisable to place a catheter in the abdominal cavity to facilitate postoperative intraperitoneal injection of anticancer drugs or radioactive colloidal gold or colloidal phosphorus.
