1. Pregnancy

If you only have abdominal pain but no period, the first possibility is pregnancy. Early pregnancy pain is usually cyclical pain or needle-like or stabbing pain on one or both sides of the abdomen. 2. Ovulation

4. Perimetrism

If a woman in her 40s has irregular menstruation and frequent abdominal pain, she may be in menopause. During menopause, many women do not have their periods for several months, but experience period pain during their periods, as well as decreased libido, lack of sleep, and weight gain.

5. Ovarian cysts

Sudden, severe abdominal pain may be caused by a ruptured ovarian cyst and requires immediate hospital admission. Ovarian cysts may cause severe pain below the belly button, over the ovary on the side with the cyst, or in the lower back or thigh.

6. Ovarian cancer

Ovarian cancer can cause abdominal or pelvic cramping, along with pain in the legs and lower back. It may be mild at first but worsen over time, with symptoms including abdominal bloating, constipation, and frequent urination.

7. Pelvic Inflammatory Disease

Pelvic inflammatory disease is a bacterial infection that can be contracted through sexual intercourse, and it can even be caused by normal bacteria colonizing in the vagina. Lower abdominal cramps are its most prominent symptom and can occur at any time of the month.
