TCM Dialectical Treatment of Ovarian Cyst

Women's estrogen needs to be secreted through the ovaries. Etiology and pathogenesis

Pei Zhengxue believes that ovarian cysts are often related to pelvic infection, and are often clinically accompanied by a series of symptoms such as cervicitis, adnexitis, pelvic inflammatory disease, and menstrual disorders. The infection can cause a series of pathological changes such as epithelial inflammatory hyperplasia, exudation, and encapsulation, thus forming ovarian cysts.

Clinically, the disease is manifested by lower abdominal pain, discomfort, increased yellow leucorrhea, odor, and may be accompanied by menstrual disorders. It is common to feel a spherical mass (cystic or solid) on one or both sides of the lower abdomen with a smooth surface, which may be accompanied by pain during sexual intercourse. If the cyst gradually grows to fill the pelvis and abdominal cavity, compression symptoms may appear, such as frequent urination, constipation, and shortness of breath. When cysts affect hormone secretion, symptoms such as irregular vaginal bleeding may occur, and in severe cases, infertility may occur.

This disease is mostly caused by insufficient body's positive energy, which allows wind, cold, dampness and heat to invade the body. It may also be caused by internal injuries from emotions, sexual intercourse, diet, organ disorders, and qi stagnation, which lead to tangible evils such as blood stasis, phlegm, dampness, etc., which condense and stay in the lower abdomen and gradually accumulate. Due to the long course of the disease, the body's vital energy is weak, and qi, blood, phlegm and dampness affect each other, causing qi disorder.

Western medical treatment mainly uses antibiotics to prevent infection, surgical treatment or ultrasound-guided puncture and extraction of cyst fluid for sclerotherapy, but it often has recurrence and sequelae, and has irreversible damage to the ovaries, which can cause a decrease in ovarian reserve function and even premature ovarian failure. Due to the above-mentioned disadvantages, more and more patients begin to seek and rely on traditional Chinese medicine to treat this disease.

Treatment of the disease is inseparable from promoting blood circulation and eliminating symptoms

Pei Zhengxue attached great importance to the statement in Golden Chamber: Pulse and Symptoms of Pregnancy Diseases in Women and Their Treatment, "If a woman has had a long-standing disease and her menstruation has not stopped for less than three months, but she has continuous bleeding and the fetus moves above the navel, this is a chronic disease... If the bleeding does not stop, it is because the symptoms have not been eliminated. To eliminate the symptoms, Guizhi Fuling Pill is the main treatment." He believed that this was the key to treating this disease.

Therefore, when he diagnosed and treated ovarian cysts based on syndrome differentiation, he always used Guizhi Fuling Pills for each type. His specific experience is as follows.

Liver depression and Qi stagnation type: There is a lump in the lower abdomen, which is not hard and can be moved by pushing. It sometimes gathers and sometimes disperses, and it may go up or down. The pain is felt from time to time and has no fixed location. There is fullness in the lower abdomen, chest tightness and discomfort, depression, irregular menstruation, red tongue, thin coating, and deep and stringy pulse. Treatment principles: Soothe the liver and relieve depression, promote qi circulation and disperse stagnation. Prescription: Guizhi Fuling Pills combined with Xiaoyao Powder with added ingredients.

Blood stasis type: There is a lump in the lower abdomen, which is hard and fixed, painful and resistant to pressure, dull skin, dry mouth and no desire to drink, delayed or continuous menstruation, dull complexion, dark purple tongue, thick and dry tongue coating, and deep, sluggish and strong pulse. Treatment principles: activate blood circulation, remove blood stasis, disperse nodules and eliminate symptoms. Prescription: Guizhi Fuling Pills combined with Shaofu Zhuyu Decoction with modifications.

Toxic-heat type: There is a mass in the lower abdomen that refuses to be pressed, pain in the lower abdomen and lumbosacral region, a large amount of yellow or mixed vaginal discharge, which may be accompanied by early or prolonged menstruation, heavy menstrual blood, aggravated abdominal pain before menstruation, irritability, fever and thirst, constipation and yellow urine, red tongue, yellow and greasy tongue coating, and a stringy, slippery and rapid pulse. Treatment principles: detoxify, remove dampness, break up blood stasis and eliminate symptoms. Prescription: Guizhi Fuling Pills combined with Taoren Chengqi Decoction with modifications.
