1. What are the causes of thick endometrium?

The causes of endometrial thickening can be divided into physiological and pathological. Under normal circumstances, the endometrium has a period of proliferation every month. If for some reason it is stimulated by estrogen for a long time, symptoms such as irregular vaginal bleeding will occur. Generally, the endometrium begins to thicken gradually from the 5th day of the menstrual cycle. On the 15th day after menstruation, under the action of progesterone secreted by the ovaries after ovulation, the already thickened endometrium continues to thicken, and the glands further increase in size and curvature, secreting mucus. At this time, the endometrium becomes thick and soft, storing necessary secretions, ready to allow the fertilized egg to implant at any time.

If a woman's hypothalamus-pituitary-ovarian axis is out of balance, or she is affected by diseases such as polycystic ovary syndrome, the endometrium may be continuously affected by estrogen for a long time, leading to anovulation. At this time, due to the lack of progesterone opposition, the endometrium lacks the transformation of the cyclical secretory phase and will be in a state of hyperplasia for a long time, causing abnormal hyperplasia of the endometrium.

2. What to do if the endometrium is thick

First, curettage treatment for women in the reproductive period: Generally, one curettage can control bleeding. If bleeding still occurs after curettage, hysteroscopy and B-ultrasound examination should be performed to rule out submucosal fibroids or other organic lesions.

Second, the menopausal transition period: It is often anovulatory functional uterine bleeding. If the menstruation is infrequent and the blood volume is heavy or the bleeding time is long after curettage and hemostasis, progesterone treatment should be given every two months, and follow-up observation should be conducted after 3 cycles in total.

Third, in the late menopause: you should ask whether to use estrogen replacement therapy alone. After curettage, you can suspend the replacement therapy or add progestin. Progestogens can inhibit the proliferation of the endometrium caused by estrogen.
