1. Threatened abortion refers to a small amount of vaginal bleeding before 28 weeks of pregnancy, followed by paroxysmal lower abdominal pain or back pain. Gynecological examination shows that the cervix is ​​not dilated, the fetal membranes are not ruptured, the products of conception have not been expelled, the size of the uterus is consistent with the number of weeks of amenorrhea, and there is hope of continuing the pregnancy. After rest and treatment, if the bleeding stops and the lower abdominal pain disappears, the pregnancy can continue. If the vaginal bleeding increases or the lower abdominal pain worsens, miscarriage may become inevitable.

2. Inevitable miscarriage means miscarriage is inevitable. It develops from threatened abortion, at which time the amount of vaginal bleeding increases, the paroxysmal lower abdominal pain worsens, or vaginal discharge (rupture of membranes) occurs. Gynecological examination shows that the cervix is ​​dilated, and sometimes embryonic tissue or fetal sac can be seen blocking the cervix. The size of the uterus is consistent with the number of weeks of amenorrhea or slightly smaller.

3. Incomplete abortion refers to the situation where the products of pregnancy have been partially expelled from the body, but some of them remain in the uterine cavity, which develops from inevitable abortion. Because some products of pregnancy remain in the uterine cavity, uterine contraction is affected, resulting in continuous uterine bleeding and even hemorrhagic shock due to excessive bleeding. Gynecological examination showed that the cervix was dilated and blood was constantly flowing out of the cervix. Sometimes placental tissue could be seen blocking the cervix or some products of pregnancy had been discharged into the vagina while some remained in the uterine cavity. The uterus is usually smaller than the number of weeks of menstruation.

4. Complete abortion means that all the products of pregnancy have been expelled, vaginal bleeding has gradually stopped, and abdominal pain has gradually disappeared. Gynecological examination showed that the cervix was closed and the uterus was close to normal size.

In addition, there are three special circumstances of spontaneous abortion:

1. Missed abortion refers to a situation where the embryo or fetus has died and is retained in the uterine cavity and has not been expelled naturally. After the death of the embryo or fetus, the uterus no longer expands but shrinks, and the early pregnancy reaction disappears. If the pregnancy reaches the second half, the pregnant woman's abdomen will not increase in size and the fetal movement will disappear. Gynecological examination showed that the cervix was not dilated, the uterus was smaller than the number of weeks after menopause, and the texture was not soft. The fetal heartbeat was not heard.

2. Habitual miscarriage refers to spontaneous miscarriage that occurs three or more times in succession. In recent years, recurrent spontaneous abortion has been commonly used to replace habitual abortion internationally. Each miscarriage usually occurs in the same month of pregnancy, and its clinical course is the same as that of general miscarriage. The causes of early miscarriage are often luteal insufficiency, hypothyroidism, chromosomal abnormalities, etc. The most common causes of late miscarriage are relaxation of the internal cervical os, uterine malformations, uterine fibroids, etc. The internal os of the cervix becomes loose after pregnancy, usually in the second trimester, as the fetus grows, the amniotic fluid increases, the pressure in the uterine cavity increases, the fetal sac protrudes toward the internal os of the cervix, and the cervical canal gradually shortens and expands. Most patients have no subjective symptoms, and once the amniotic membrane ruptures, the fetus is expelled quickly.

3. Abortion infection During the abortion process, if vaginal bleeding lasts too long, tissue remains in the uterine cavity, or an illegal abortion is performed, it may cause intrauterine infection. In severe cases, the infection can extend to the pelvic cavity, abdominal cavity, and even the whole body, with complications such as pelvic inflammatory disease, peritonitis, sepsis, and septic shock, which is called abortion infection.
