What are the pathological manifestations of chronic pelvic inflammatory disease?

Pelvic inflammatory disease can actually be divided into chronic and acute types. 1. Chronic endometritis

Most of them occur after childbirth, abortion or cesarean section, because the placenta, fetal membrane retention or poor uterine involution are very susceptible to infection. It can also be seen in elderly women after menopause. Because the endometrium is thin, it is easy to be infected by bacteria. In severe cases, the cervical canal adhesions form uterine cavity abscesses. The endometrium is congested and edematous, with interstitial inflammatory cell infiltration.

2. Chronic salpingitis, hydrosalpinx, salpingo-oophoritis and tubo-ovarian cysts

Chronic salpingitis is the most common and is usually bilateral. The fallopian tubes become thickened, fibrotic and adherent to surrounding tissues. When the fimbria and isthmus of the fallopian tube are closed due to adhesions caused by inflammation, or when pus accumulates in the fallopian tube, the pus is absorbed, and serous exudate accumulates, hydrosalpinx is formed. Inflammation of the fallopian tubes spreads to the ovaries and adhesions occur, forming fallopian tube oophoritis. The fimbria of the fallopian tube adheres to and penetrates the ovary, and fluid seeps out to form a fallopian tube-ovarian cyst. It can also be caused by the absorption of pus from a fallopian tube-ovarian abscess.

3. Chronic pelvic connective tissue inflammation

The connective tissue around the uterus proliferates, becomes thicker and harder, and in severe cases the uterus may become fixed and its movement restricted.
