Primary dysmenorrhea begins at menarche and the patient will feel a distension and cramping pain in the lower abdomen every time menstruation comes. In severe cases, it is accompanied by nausea and vomiting. The pain area may radiate to the back and inner thighs. The pain lasts for 48 to 72 hours. Even though the menstrual flow is heavy and there are blood clots and tissue, the pain will only be relieved after the discharge. Generally, the discharged tissue is pink, flaky, and of varying sizes. Sometimes there are whole triangular tubular objects. This is caused by the endometrium shedding in one piece. This tissue is called decidual tubular type. This type of dysmenorrhea is also called membranous dysmenorrhea. Transrectal examination (for unmarried persons) showed no obvious positive signs. If the history of dysmenorrhea is long, it may be found that the uterus has poor mobility, masses are formed in the bilateral adnexa or nodules in the posterior depression, and there is tenderness. She may suffer from endometriosis or ovarian chocolate cyst, which often presents as secondary dysmenorrhea.
