Ectropion exposes the cervical columnar epithelium at the external os of the cervix, which appears red and rough. The red color is because the columnar epithelium is arranged in a single layer with a rich vascular network underneath. The roughness is because the columnar epithelium fuses with each other to form villi or granular shapes.
