Different types of vaginitis have different clinical manifestations, which are briefly introduced as follows: Bacterial vaginosis 1. Symptoms: 10% to 50% of patients with this disease are asymptomatic. Those with symptoms often complain of grayish white leucorrhea with a fishy odor, vaginal burning and itching. 2. The secretions can be easily wiped off the vaginal wall, and the vaginal mucosa may not be congested or red and swollen. 3. Common complications occur simultaneously with gynecological cervicitis, pelvic inflammatory disease, and often with Trichomonas vaginitis. In addition, bacterial vaginosis during pregnancy can often cause adverse perinatal outcomes, such as chorioamnionitis, amniotic fluid infection, premature rupture of membranes, premature birth, and endometrial infection after cesarean section or vaginal delivery. Trichomonas vaginitis 1. Trichomonas vaginitis is caused by Trichomonas vaginalis. The most common symptom is an increase in leucorrhea, which is yellow-green purulent, foamy, and has a special odor. It can be a thin serous fluid. Sometimes vaginitis is mixed with blood. 20% of leucorrhea contains foam. The vulva is itchy and in severe cases the leucorrhea may be mixed with blood. Burning and pain during sexual intercourse are also common. When the infection involves the urethra, there may be urinary pain, urgency, and even hematuria. Patients should pay attention to personal hygiene and avoid unclean sexual intercourse and cross infection. 2. Physical signs Examination of this disease may reveal congestion and edema of the vaginal and cervical mucosa, often with scattered red spots or strawberry-shaped protrusions, and a large amount of leucorrhea in the posterior fornix. 3. Common complications: Trichomonas can consume glycogen in the epithelium, change the pH value in the vagina, and hinder the growth of lactobacilli, so it is easy to cause secondary bacterial infection. At this time, the leucorrhea is grass green and has a foul odor. Candidal vaginitis 1. Symptoms: The most common symptoms are excessive vaginal discharge, burning and itching of the vulva and vagina. It may affect the urethra and cause frequent urination. Symptoms such as urinary urgency and pain when urinating. 2. Physical signs: Typical leucorrhea is curd-like or flaky. The vagina and vaginal vestibule mucosa are highly edematous and covered with a white curd-like film, which is distributed in dots or sheets and is easy to peel off. Underneath is a damaged, flushed base, which may form ulcers or leave ecchymosis. In severe cases, the labia minora may swell and adhere. However, not all leucorrhea has the above typical characteristics. It can range from watery to curd-like leucorrhea. Some are completely thin and clear serous exudates, which often contain white flakes. Senile vaginitis 1. Symptoms: The main symptom is an increase in leucorrhea, which is mostly yellow watery. In severe cases, it may be purulent and have a foul odor, sometimes light bloody, and even a small amount of vaginal bleeding may occur. It is often accompanied by a feeling of heaviness in the lower abdomen and vagina or the vaginal skin is affected by inflammatory secretions, which may cause mild itching. 2. Physical signs: The vagina shows senile changes, with disappearance of wrinkled walls, thinning of the epithelium, congestion of the mucosa, and scattered punctate congestion on the surface. In severe cases, the epithelium may peel off to form superficial ulcers, and the cervix may also have punctate congestion. If senile vaginitis does not heal for a long time, the submucosal connective tissue will become fibrotic, the vaginal elasticity will disappear, and the vagina will become narrow, especially the vault area, making it difficult to expose the cervix. Childhood vaginitis Purulent discharge may be seen at the vaginal opening. The child cries incessantly due to pain and itching in the vulva, becomes irritable and restless, and often scratches the vulva with his hands. Inspection revealed that the mucous membranes of the vulva, clitoris, urethral orifice, and vaginal orifice were congested and edematous, and purulent secretions were flowing out of the vaginal orifice. In severe cases, ulcers can be seen on the surface of the vulva and adhesions can be seen on the labia minora. The adhered labia minora cover the vaginal opening and urethral opening, leaving only a small hole above and below, through which urine is discharged. 1) Causes The main reasons why young girls suffer from vulvovaginitis are: ① Anatomically, due to the lack of estrogen in young girls, their vulva is immature, and they lack the protection of labia majora and minora fat and the defense of pubic hair against microorganisms. In addition, the skin of the vulva is particularly thin and tender, making it easy for them to be invaded by pathogens. They are also sensitive to invasion by microorganisms and are easily injured and scratched. ②The pH value of a young girl’s vagina is neutral, which is suitable for the growth and reproduction of pathogens. ③ Children have poor hygiene habits and are prone to getting dirt, sand and other dirty things into the vagina. Or out of curiosity, they may put foreign objects such as rubber and glass balls into the vagina, causing inflammation. ④ Children who are sexually abused may suffer from vulvar and vaginal injuries and STDs such as gonorrhea infection. Common pathogens include Staphylococcus, Streptococcus, Escherichia coli, Trichomonas, Candida, Gonorrhea, Chlamydia, herpes virus and HPV. Menstrual vaginitis It is mostly caused by not paying attention to menstrual hygiene during menstruation, especially using unclean menstrual products, which causes the vulva to be contaminated by unclean objects.
