Ultrasound examination for mastitis

Mastitis often occurs in women during the postpartum lactation period, and is more common in primiparas. When the breast becomes infected 3 to 4 weeks after delivery, it causes acute suppurative mastitis. The early stage of inflammation is a nodule. After suppuration, the nodule becomes soft and ruptures, or after incision and drainage of pus, the inflammation gradually subsides and the disease heals. Patients often have symptoms such as high fever, chills, redness, swelling, pain, and heat in the breasts. The inflammation is mostly located in the outer and lower part of the breast.

Breast swelling and tenderness. Laboratory tests: elevated white blood cell count, etc. After anti-inflammatory treatment, the inflammation subsides, or after liquefaction incision and drainage of pus, the symptoms disappear. Acute suppurative mastitis can usually be diagnosed by clinical observation and palpation. The most commonly used test is blood count, which can detect elevated white blood cells or neutrophils. Color Doppler ultrasound can determine the location and size of the abscess cavity.

If the abscess is located deep and the abscess cavity is located in the post-glandular space, the redness and swelling of the skin are often not obvious. At this time, puncture and extraction of pus are required for confirmation. If the treatment is improper, the abscess forms slowly, the local lump does not disappear, the skin redness and swelling and systemic symptoms are not obvious, and chronic inflammation occurs, which needs to be differentiated from other diseases.

Clinical significance

Color Doppler blood flow imaging, as a non-invasive detection method, is of great value in the diagnosis and differentiation of mastitis. High-frequency ultrasound has a high diagnostic rate in the chronic stage of mastitis, but sonograms lack specificity in the acute and subacute stages and must be combined with other examinations and clinical history.

Ultrasound shows: In case of mastitis, if it has not yet become purulent, the breast will show local thickening, unclear boundaries, enhanced echoes, and color ultrasound: abundant blood flow. When an abscess forms, the interior appears as an inhomogeneous anechoic area. Ultrasonic diagnosis of mastitis is generally not difficult. However, this disease should be differentiated from breast cysts (duct ectasia). The latter is asymptomatic, with only duct dilation. Sometimes, it should also be differentiated from breast cancer. The latter has a nodule, but the symptoms are not obvious.
