Stage II: Radical hysterectomy, bilateral salpingo-oophorectomy, and pelvic and para-aortic lymph node dissection should be performed.

Stage III: Comprehensive treatment is the main approach, with radiotherapy often being the first choice, or surgery being performed after the cancer has shrunk through radiotherapy; postoperatively, radiotherapy, chemotherapy, and hormone combined treatment are used as adjuvants. For patients with distant metastasis, radiological interventional therapy may be performed as appropriate.

Stage IV: progestin therapy and/or chemotherapy.

2. Radiation therapy

Radiotherapy alone: ​​suitable for patients in the advanced stage or who cannot tolerate surgery. It includes intracavitary and extracorporeal irradiation.

(1) Intrauterine irradiation: radiotherapy within the uterine cavity and vaginal vault.

(2) External irradiation: The irradiated area includes the tumor and pelvic infiltration.

3. Surgery plus radiotherapy

(1) Preoperative radiotherapy: It can reduce the recurrence of vaginal vault, shrink or eradicate regional lymph node metastasis, reduce the size of the tumor, inactivate cancer cells, reduce intraoperative spread and bleeding, and reduce the possibility of postoperative recurrence and distant metastasis, thus improving the surgical cure rate. Generally, intracavitary radiotherapy is performed, and surgery is performed 7 to 10 days after intracavitary radiotherapy, and preoperative external irradiation is performed 6 weeks after radiotherapy.

(2) Postoperative radiotherapy: supplements the deficiencies of surgery. If postoperative pathology confirms that cancer cells have involved the cervix, the cancer has invaded more than 1/2 of the uterine myometrium. Poor cell differentiation. The lesion extends beyond the uterus, and the pathological examination shows adenosquamous carcinoma/clear cell carcinoma/papillary adenocarcinoma, and the parauterine lymph nodes are positive. External irradiation should be supplemented after surgery. Intracavitary radiotherapy can be performed 2-4 weeks after surgery.
