3. The most common symptom of facial muscle spasm is twitching of the muscles at the corners of the mouth, which manifests as irregular, rapid, and paroxysmal twitching. In the early stage of the disease, the convulsions are mild and last for a short time, just a few seconds. Later, they gradually last for several minutes or longer, but the intervals will gradually shorten and the convulsions will become more frequent and severe.

In severe cases, the condition may present as rigidity, leading to emotional tension, inability to speak, mouth corners tilted to the same side, fatigue, inability to open the eye on the same side, relief when quiet, disappearance during sleep, and aggravation when emotionally excited. A small number of patients may experience mild facial pain during convulsions, and some patients may also have tinnitus and ipsilateral headache. Neurological examination: no positive signs, except paroxysmal twitching of facial muscles.

5. In a few cases of hemifacial spasm, symptoms may be accompanied by mild facial muscle paralysis on the affected side in the late stages. In a very small number of patients with hemifacial spasm, the symptoms often occur on both sides one after the other, with one side twitching more severely than the other.
