Regular menstrual cycles, significantly increased menstrual volume (more than 200 ml at a time), pelvic examination excluding organic lesions, and bidirectional basal body temperature are called menorrhagia. In addition to heavy menstrual flow, other symptoms include thirst, irritability, restless sleep, shortness of breath, laziness to speak, limb weakness, dizziness, tinnitus, and hot hands and feet.

The cause of menorrhagia is not very clear at present. Because the levels of estradiol and progesterone in the blood are not significantly different from those of women with normal menstruation, most scholars believe that this disease is related to the imbalance of the prostaglandin (PGS) series products in the endometrium. Under normal circumstances, prostaglandin E2 (PGF2) and prostaglandin F2α (PGF2α) as well as prostaglandin I2 (PGI2) and thyroxine A2 (TXA2) of the endometrium are in a relative equilibrium state. The former two mainly regulate the tension and contraction function of the uterine muscles, while the latter two mainly regulate vascular contraction and relaxation, platelet aggregation and thrombus formation, thereby leading to excessive menstrual bleeding. This disease is also accompanied by an increase in plasmin, which leads to an increase in fibrin degradation products (FDP), obstruction of fibrin deposition, inhibition of the closure of the intimal blood vessels and the coagulation process, and causes excessive menstrual bleeding.

Excessive menstruation can occur after childbirth, abortion, IUD insertion, and tube ligation. Some of them are anovulatory menorrhagia during adolescence, and the above are generally functional menorrhagia. Excessive menstruation can be secondary to organic diseases, such as uterine fibroids, uterine adenomyosis, endometriosis, endometrial polyps, endometritis, salpingitis, etc. caused by local lesions; systemic diseases, such as patients with incomplete compensation of heart function, who suffer from excessive menstrual flow due to venous congestion; liver disease, especially cirrhosis, which affects the excretion of estrogen and causes endometrial hyperplasia, leading to increased menstruation; blood diseases, such as idiopathic thrombocytopenic purpura, leukemia, iron deficiency anemia, etc., which can also cause increased menstrual flow due to disorders in the blood coagulation mechanism. Therefore, the causes of increased menstrual volume are complex and need to be identified and taken seriously.
