The urgency and necessity of stroke prevention and treatment

Stroke is the leading cause of death in my country. It is characterized by high morbidity, high disability rate, high recurrence rate, high economic burden, and high mortality rate. Stroke is preventable and treatable, but the incidence and disability rates of stroke remain high due to the neglect of risk factors and lack of understanding of stroke emergency measures and effective time windows.

Urgency: After a stroke occurs, 1.9 million brain cells die every minute; for every hour of delay, the brain ages by 3.5 years. The most effective treatment measures are "intravenous thrombolysis" and "interventional thrombectomy", with treatment time windows of 4.5 hours and 6 hours respectively. According to statistics from the stroke center of our hospital, the average time to hospital after a stroke among local residents is 18 hours, and 86% of patients miss the best time for treatment, which seriously affects the prognosis and leads to disability.

Necessity: The National Brain Protection Committee requires a 32-word policy of "moving the checkpoints forward and lowering the center of gravity; improving literacy and prioritizing education; interdisciplinary collaboration and standardized diagnosis and treatment; high-risk screening and targeted intervention", which is our work goal.

To reduce the incidence and recurrence rates: the stroke prevention and control threshold should be moved forward, and stroke screening and health education should be carried out; stroke prevention and control knowledge should be popularized among the whole people, the scope and intensity of publicity should be increased, and the prevention and control awareness of the general public should be improved; a county-level medical alliance stroke prevention and control network should be established, and coordination between the upper and lower levels and tiered diagnosis and treatment should be carried out, and the network should be used for tertiary prevention.

To reduce disability and mortality rates: Promote the knowledge of early stroke identification such as "BEFAST" and "Stroke 120", so that stroke patients can reach the stroke center within the time window for thrombolysis and thrombectomy, effectively reducing disability. At the same time, vigorously improve stroke emergency treatment technology and promote appropriate technology.

Suggestions for promoting stroke prevention and treatment projects:

1. Implement relevant national policies. In June this year, 10 departments including the National Health Commission, the Ministry of Education, the Ministry of Industry and Information Technology, the Ministry of Finance, the Ministry of Transport, the Ministry of Commerce, the State Administration of Radio, Film and Television, the National Medical Insurance Administration, the State Administration of Traditional Chinese Medicine, and the State Drug Administration decided to implement the project of strengthening stroke prevention and treatment to reduce one million new disability cases, and jointly formulated the "Comprehensive Plan for Strengthening Stroke Prevention and Treatment to Reduce One Million New Disability Cases", which put forward requirements: implement comprehensive stroke prevention and control, carry out in-depth health education, continue to carry out screening and intervention for high-risk groups, promote appropriate technologies, strengthen emergency first aid for stroke, standardize treatment and rehabilitation follow-up, further improve the prevention and treatment effect of stroke, and effectively reduce the incidence and disability rate. Our goal is to implement the "One Million Disability Reduction" plan.

2. It is recommended that the government take the lead, multiple departments participate, divide the work and cooperate to jointly contribute to the health of the people.

① Government: Build a full-cycle stroke management platform for county-level medical alliances, and provide relevant policy support for projects such as screening and prevention of high-risk groups and construction of stroke centers.

② Health administrative departments: Work with relevant departments to organize and implement the "Comprehensive Plan to Strengthen Stroke Prevention and Control to Reduce Millions of New Disabilities" and carry out supervision and evaluation, and guide medical and health institutions to implement health service measures.

③Financial departments: Implement investment policies as required, and provide financial support for screening and intervention programs for people at high risk of stroke.

④ Medical insurance departments: Promote the volume-based procurement of drugs and high-value consumables, exchange volume for price, reduce prices, and lighten the burden on the people. Improve the protection and payment policies for stroke thrombolysis and thrombectomy.

⑤ Commerce and transportation departments: Set up stroke center publicity and guide signs at highway intersections and transportation hubs in the city; use buses and taxis to promote the stroke green life channel; set up self-service blood pressure testing points in large shopping malls, supermarkets, stations and other public places to increase the public's awareness of hypertension.

⑥ Radio and television propaganda departments: Use radio, television, newspapers, and new media to carry out regular and various forms of popular science publicity on stroke prevention, treatment, and early identification.

⑦ Ministry of Education and Ministry of Commerce: Promote scientific knowledge on stroke prevention, treatment and early identification to primary and secondary school students, high school students and vocational high school students; and popularize relevant knowledge to employees of enterprises and institutions.

All departments should work together to create a good atmosphere. We should strengthen positive publicity, public opinion supervision, scientific guidance and typical reports, enhance the general public's awareness of the project of strengthening stroke prevention and treatment to reduce the number of newly diagnosed disabled people, and form a good atmosphere of concern, support and joint participation from the whole society.
