Is 25mm of pelvic fluid a pelvic inflammatory disease?

Three common causes of physiological pelvic effusion

1. The defensive function of the pelvic peritoneum

The pelvic cavity is the female internal reproductive organs and the connective tissue surrounding them. The pelvic peritoneum is a layer of mucous membrane that covers most of the pelvic organs. Under normal circumstances, the peritoneum secretes a small amount of serous fluid to reduce friction between abdominal organs. The peritoneum also has a defensive function, and the peritoneal fluid contains white blood cells and some antibodies. Because the female abdominal cavity is connected to the outside of the abdomen through the fimbria of the fallopian tube, external pathogens and even tiny particles from the outside may enter the abdominal cavity through the vagina - cervix - uterine cavity - fallopian tube. In order to defend against the invasion of foreign objects, the female abdominal cavity needs to secrete a certain amount of peritoneal fluid to fight against foreign objects invading from the outside.

2. Follicular fluid after ovulation

The surface of the ovaries is not covered by peritoneum. The ovaries ovulate every month, and when the follicles rupture, the follicular fluid is discharged into the abdominal cavity and accumulated in the rectouterine fossa, forming a few milliliters at least and more than 10 milliliters at most.

Tips The full medical name of the rectouterine pouch is rectouterine pouch, also known as Douglas pouch. It is the lowest point in the female pelvis and the most common place for fluid accumulation.

3. Backflow of menstrual blood

When every woman has her menstrual period, menstrual blood can also flow back from the uterine cavity into the fallopian tube, and then "flow back" into the abdominal cavity.

Generally speaking, it is normal for B-ultrasound to detect 20 to 30 mm of "fluid accumulation" in the rectouterine fossa. These "effusions" may be normal abdominal secretions (used as a defense), follicular fluid after ovulation, or a small amount of menstrual blood that flows back. Physiological pelvic effusion, most women have no obvious clinical symptoms.
