HCG diagnosis of early pregnancy The early diagnosis of ectopic pregnancy is mainly based on the detection of blood HCG (human chorionic gonadotropin). Because HCG is a specific hormone secreted during pregnancy, β-HCG can be used to assist in the diagnosis of early unruptured ectopic pregnancy. Normally developed villi secrete a large amount of HCG, and the daily titer continues to rise rapidly, increasing by more than 66% every 48 hours. That is, if the increase in β-HCG is greater than 66% every two days, it can be diagnosed as an intrauterine pregnancy; if the increase is less than 66%, there is a high possibility of ectopic pregnancy or intrauterine pregnancy hypoplasia. After a woman becomes pregnant, an increase in β-HCG can be detected in her blood from the 9th to 11th day, and the amount of β-HCG can double every two days thereafter (even if there is threatened miscarriage, the rate of HCG increase will not change). For example, if today's value is 234, if the test result the day after tomorrow is around 450, it can be considered as normal intrauterine pregnancy. If the rate of increase is slow for two consecutive times, it indicates an ectopic pregnancy or abnormal development of the embryo. For example, if it is 10 today, 15 the day after tomorrow, and 17 two days later, such HCG values are definitely abnormal, and the success rate of preserving the pregnancy is extremely low. If the HCG value continues to drop significantly, it is best to perform a uterine curettage even if the fetal heart rate is detected by B-ultrasound, as this indicates that the fetus is actually brain dead. If HCG increases very quickly, it indicates the possibility of hydatidiform mole and must be closely monitored. Of course, it could also be twins. HCG measurement precautions 1. Differentiate between normal and abnormal pregnancy: HCG changes during pregnancy present a bimodal curve. According to the normal HCG concentration changes, if the first HCG is within the normal range, 79% of people can have a full-term pregnancy. If the first HCG is lower than normal, 92% of pregnancies will fail. 2. Diagnosis and treatment of miscarriage: In incomplete miscarriage, there is still placental tissue remaining in the uterus, and HCG is positive. HCG may be negative in complete miscarriage or stillbirth. If HCG is below 2500U/L and gradually decreases, miscarriage or stillbirth is possible; when it drops to 600U/L, miscarriage is inevitable. If HCG in urine continues to decrease, it means that the fetus preservation is ineffective, otherwise it indicates that the fetus preservation is successful. Serum HCG should return to normal 4 days after delivery or 13 days after artificial abortion. If this situation is not met, the possibility of abnormality should be considered. 3. In cases of malignant hydatidiform mole, choriocarcinoma and male testicular teratoma, the urine HCG content is very high, reaching 100,000 to millions of international units. 4. Others: HCG levels vary during pregnancy toxemia and are often high; HCG levels increase during multiple pregnancies.
