How to care for episiotomy and lacerations

Things to note when performing episiotomy:

Limited use of episiotomy does not result in more adverse consequences, such as vaginal or perineal injury, pain, dyspareunia, or urinary incontinence. The only disadvantage is an increased risk of early perineal laceration.

For some mothers whose conditions allow them to give birth smoothly without episiotomy, this will add some unnecessary pain. Moreover, the episiotomy wound may heal quickly physically, but the psychological impact on some mothers will be lingering.

Episiotomy is a routine operation during vaginal delivery and is only used in limited cases. However, it accounts for about 90% of vaginal deliveries in China, while the proportion abroad is about 20%.

Why is episiotomy so commonly used in China? Is there a risk of perineal tear if I don’t have an episiotomy? Objectively speaking, episiotomy should not be used as a routine surgery in clinical practice, but there are many prerequisites for accurately judging whether a parturient needs an episiotomy. A very important part is that current obstetricians and nurses need a strong sense of responsibility, superb medical skills, rich clinical experience and judgment, as well as more careful care and nursing for parturients. However, the current medical level and treatment environment in most parts of our country cannot meet these conditions. The time is not yet ripe to implement limited episiotomy. Blindly demanding to reduce the episiotomy rate will only do more harm than good, for example, increasing the number of cases of perineal laceration and even threatening the health and life of the fetus.
