In traditional Chinese medicine, black pepper can treat cold phlegm, food accumulation, cold pain in the abdomen, nausea, vomiting of clear water, diarrhea, cold dysentery, and can also be used to detoxify food poisoning. Many writings after the 5th century also recommended the use of pepper to treat eye diseases, usually by applying an ointment made from pepper directly to the eyes. There is no evidence in modern medicine that these treatments are effective; applying pepper directly to the eyes is uncomfortable and may cause damage to the eyes.

Pepper was long believed to cause people to sneeze; this is still the case today. Some people believe that peppermint irritates the nostrils and causes sneezing; others attribute it to the pepper powder, while others feel that pepper is not a very effective sneeze-inducing agent. However, few controlled experiments have been conducted to answer this question.

Because pepper can irritate the intestines, it should not be included in the diet of patients who have undergone abdominal surgery or have abdominal ulcers: milder foods are generally used instead.
