Gonorrhea is the most common disease of the urinary and reproductive system. It is also a sexually transmitted disease. In recent years, the incidence of gonorrhea has been increasing, especially among young people. It is also the main type of sexually transmitted diseases. Do we all know the symptoms of gonorrhea in women? Generally, the symptoms of gonorrhea in women are very mild at the beginning, and sometimes there are no symptoms.

1. Simple gonorrhea

That is, those without complications.

1. Acute gonococcal urethritis and cervicitis: The disease occurs 2 to 5 days after sexual contact. The main lesions are in the cervix and urethra. The symptoms are mostly mild, and even 60% to 80% are asymptomatic. The first symptoms are frequent urination, urgency, pain, and difficulty urinating. At the same time, there is abnormal or increased vaginal discharge, often yellow purulent leucorrhea, and a burning and itching sensation in the vulva.

During the examination, the vulva, vaginal opening and urethral opening were found to be congested, red and swollen. In acute cervicitis, the cervix becomes congested and the yellow-green purulent secretions increase.

2. Chronic gonococcal urethritis and cervicitis: It occurs 2 weeks after the acute phase and the symptoms may disappear due to untreated or incomplete treatment of acute gonorrhea. The patient complained of lower abdominal distension, back pain, and increased leucorrhea. Although the gonococci of chronic gonorrhea do not exist in the secretions of the reproductive tract, they can hide in the deep of the paraurethral glands, Bartholin's glands or cervical mucosal glands for a long time, acting as lesions and repeatedly causing acute attacks.

3. Gonorrhea during pregnancy: The incidence of gonorrhea during pregnancy is about 2.5%~7.3%, which poses a great threat to mother and baby. It can easily cause premature rupture of membranes and amniotic cavity infection, which can lead to intrauterine growth retardation and premature birth of the fetus; it can also cause postpartum endometritis and even postpartum sepsis.

4. Gonorrheal vulvovaginitis in young girls: It is transmitted through direct or indirect contact with objects contaminated with gonococci. The main symptoms are redness and swelling of the vulva and urethral orifice, which are bright red and infiltrated, and are accompanied by pain, itching or dysuria. There is yellow-green purulent discharge from the vaginal opening.

2. Gonorrhea with complications

Including gonococcal bartholinitis, paraurethral glanditis, perianal inflammation, endometritis, salpingitis, tubo-ovarian abscess and pelvic inflammatory disease.

If there is paraurethral gland inflammation, when the urethra is pressed with fingers, pus can be seen overflowing from the urethral opening. Acute infection of Bartholin's glands is often unilateral, with redness, swelling and severe pain at the gland opening. Obstruction of the gland duct may form an abscess, which is mainly salpingitis, pelvic inflammatory disease, and tubo-ovarian abscess.

Abscess rupture can cause pelvic abscess and diffuse peritonitis. 60%~70% of the cases occur during the menstrual period or within 1 week after menstruation. The onset is acute and patients experience chills, fever, increased leucorrhea and lower abdominal pain. In the later stages, there is tenesmus and diarrhea.

Physical examination: tenderness, rebound tenderness and muscle tension in the lower abdomen, cervical motion tenderness, and adnexal tenderness. When an abscess is formed, a mass can be felt and pus can be obtained by puncturing the posterior fornix. Acute salpingitis is an important complication of gonorrhea. Can cause infertility and ectopic pregnancy.

2. Complications of gonorrhea in women

Gonorrhea in women, especially gonococcal infection of the cervix, may be combined with infection of the upper reproductive system, causing more serious consequences, such as gonococcal pelvic inflammatory disease, including endometritis, salpingitis, tubo-ovarian cysts, pelvic abscess, peritonitis, etc.

(1) Patients with endometritis may experience increased vaginal discharge, lower abdominal pain, uterine enlargement and pain, and fever in acute cases.

(2) Patients with salpingitis experience fever, chills, general malaise, vomiting, and cramping pain in the lower abdomen and waist, which may radiate to the perineum. There is a lot of leucorrhea with pus and blood. There is tenderness on both sides of the lower abdomen when palpated. Small tender lumps can be felt and the uterus is also tender. If the treatment is not timely and thorough, it will become chronic salpingitis and may cause ectopic pregnancy. The inflammation of the fallopian tube may cause adhesion, water accumulation or pus accumulation, which may lead to infertility.

Everyone should understand the symptoms of gonorrhea in women. These symptoms make us feel very uncomfortable. In daily life, we should prevent the occurrence of gonorrhea in time, especially those who have sex. They should pay attention to safety and try to use condoms to avoid cross infection. At the same time, women with gonorrhea should receive timely treatment to avoid the occurrence of complications.
