What are the causes of Bartholin's gland cysts?

Bartholin's gland cyst is caused by blockage of the opening of the Bartholin's gland duct, resulting in the accumulation of secretions in the gland cavity.

Causes of Bartholin's duct obstruction include:

1. After the infected Bartholin's gland cyst dissipates, the pus is digested and absorbed, and the mucus in the gland cavity becomes viscous and blocks the gland.

2. Congenital stenosis or closure of the lumen. The gland is congenitally stenotic or closed, which results in obstruction or inability to discharge the fluid in the gland lumen, causing cysts.

3. Damage to the Bartholin's gland ducts: In some cases, the Bartholin's gland ducts may be blocked and cysts may form due to lacerations on both sides of the vagina and perineum during childbirth, damage to the glands during episiotomy, or severe scar tissue contraction after the episiotomy heals.

2. Pathogenesis

The cyst contents are transparent mucus and rarely serous. The cyst is unilocular, and light yellow transparent mucus can be extracted by puncture. Sometimes it is mixed with blood and appears bright red or reddish brown, which can easily be mistaken for endometriotic cysts, especially when the epithelial cells covering the cyst wall contain pseudoxanthomatous cells.

In the early stage of cystic fibrosis, the cyst wall covers transitional epithelial cells. As the cyst fluid increases and the pressure increases, the transitional epithelial cells may change into unilateral cubic epithelial cells or flat epithelial cells, or even shrink. If secondary infection occurs, the cyst wall will show diffuse inflammatory connective tissue and even form small granular tissue.

What are the early symptoms of Bartholin's gland cyst?

Bartholin's gland cysts vary in size, most start small and slowly expand, grow slowly, and some may persist for many years without changing. It is common to see the chest rotating, usually no larger than the size of an egg, and rarely occurs on both sides at the same time. If the cyst is small and not infected, the patient may have no active symptoms and is usually discovered during a gynecological examination. If the vagina is swollen, the patient may feel a sense of heaviness and swelling in the private parts or discomfort during sexual intercourse.

Examination showed that the cysts were mostly on one side, but could also be bilateral. The appearance of the skin is normal. The cyst is located at the location of the Bartholin's glands below the back of the vulva and between the frenulum of the labia. It is crescent-shaped, oval or circular. The cyst is significantly protruding on both sides of the labia majora, and the labia minora of the affected limb is expanded. The swelling is mobile and there is no obvious dull pain. When sexual indulgence is excessive, the swelling will expand rapidly. In the case of secondary infection, local swelling and pain are obvious, the patient has systemic symptoms such as fever, and the swelling may develop into a cyst.

Cystic masses below the labia majora on one side appear in women of childbearing age, with no obvious active symptoms, mucus on puncture, and the characteristic of sexual excitement affecting its size. Pathological examination can be diagnosed. It is generally not difficult to diagnose the cyst based on its location, appearance, or the lack of signs of inflammation. Partial puncture can be performed when necessary, and its contents can be distinguished from Bartholin's glanditis, cysts, varicose veins, etc.

Common questions before treatment of Bartholin's gland cyst?

Based on medical history, partial appearance and digital rectal examination, it is generally not difficult to make a diagnosis. However, you should also pay attention to whether there are any abnormalities in the urethral opening and paraurethral glands. Due to severe pain, vaginal speculum examination is no longer possible and can be temporarily suspended if not necessary. Generally, samples should be taken from the interface of the Bartholin's glands, the urethral orifice, and the paraurethral glands for acid-fast staining to detect pathogens. After the acute symptoms of Bartholin's gland inflammation, due to the blockage of the gland ducts, the gland secretions cannot be discharged and are retained, resulting in Bartholin's gland cysts.

There is a soybean-sized gland duct behind each labia majora of women. The gland duct has a gland body about 1 to 2 centimeters long and opens in the groove between the labia minora and the hymen. This gland duct is called "Vestibular gland" in medicine. Under normal circumstances, the Bartholin's glands are not easily palpable. When sexually aroused, the Bartholin's glands secrete yellow-white mucus, which lubricates the vulva. The so-called "Bartholinitis" means that the Bartholin's glands are infected with bacteria, causing inflammation. The general manifestations are: swelling of the glandular ducts, accumulation of exudate, and inability of inflammatory substances to be discharged, resulting in glandular duct cysts. At this time, a walnut-sized lump can be touched under the labia majora, which is partially red, swollen, hot, and painful, and may cause fever in severe cases. The treatment is to cut open the cyst and drain it. If you don't seek medical attention right away, the cyst may burst naturally. If the drainage method is not smooth, the disease will relapse. Bartholinitis mostly occurs in growing women.
