Estrogen and progesterone therapy is the first choice for the treatment of ovarian hair excess. If the patient wishes to become pregnant, ovulation should be induced, and drugs such as clomiphene, HMG, FSH, and Gn-RH can be used. If oral contraceptives containing estrogen and progestin are used, the effect will be better, because not only can progestin inhibit LH, but estrogen can also increase SHBG and reduce free testosterone, and progesterone also inhibits the activity of 5α-reductase in the skin. Preparations containing levonorgestrel, desogestrel, etc. are more effective. For example, Marvelon contains 150μg desogestrel and 30μg ethinyl estradiol (ethinyl estradiol), which is very effective.

If there are contraindications to oral contraceptives or if you do not want to use them, you can give medroxyprogesterone acetate (150 mg intramuscularly once every 3 months); or take 30 mg orally every day. Slightly less effective than birth control pills. It reduces SHBG and slightly increases free testosterone.

Drug treatment generally takes a long time to take effect, and it often takes 6 months to achieve satisfactory results. If it can be combined with physical methods, more ideal results can be achieved. It is generally believed that it can be suspended after 1 to 2 years to observe whether ovulation resumes. If ovulation occurs, treatment should be stopped. If ovulation still does not occur, androgen suppression can continue for 6 months to 2 years, but the disease will often relapse eventually. It is best to repeat the medication before relapse.
