The skin surface of the vulva from the mons pubis to the outside of the labia majora is covered with pubic hair. Because the vulva is easily contaminated with urine, feces, and leucorrhea, the vulva skin is often in a humid environment, which is suitable for bacterial growth. Scratching, friction, high temperature, humidity and sweating are the triggering factors of this disease. At this time, if the vulvar skin is abraded, or if hygiene is not paid attention to, or the body's resistance is reduced, the hair follicles and surrounding tissues may be infected by staphylococci and other bacteria, and the bacteria may invade the hair follicles and their sebaceous glands, causing acute suppurative infection. The pathogens are mostly Staphylococcus aureus, Staphylococcus epidermidis and Staphylococcus albus. It is more common after skin preparation before surgery. This disease can be caused by decreased body resistance, local uncleanliness of the vulva, or damaged skin friction caused by obesity. The skin around the hair follicles of the vulva becomes red, swollen and painful, gradually forming a cone-shaped pustule with a pubic hair protruding from its center. Pustules may be multiple, and adjacent small pustules may fuse together to form large pustules, accompanied by severe congestion, edema and pain in the vulva. At the beginning, there is pain at the opening of the hair follicle and its surrounding area, the skin is slightly red, and gradually red blisters appear and are tender. There is a hair sticking out from the top of the red blister. If there is pus, a small white dot may appear on the top of the red blister. The pus head gradually increases in size and becomes cone-shaped. After a few days, the central tissue of the nodule becomes necrotic and softens, and a small yellow pus plug appears. After a few more days, the pus plug falls off, and pus is discharged. The inflammation gradually subsides, but it often recurs. There can be one or more hair follicles infected. If the inflammation gets worse, a boil may form.
The treatment of vulvar folliculitis is not difficult. First of all, the vulva should be kept clean and dry. You can use 1:5000 potassium permanganate solution or Chinese medicine for clearing heat and detoxification for sitz bath, and then apply antibiotic ointment or Sihuang ointment or Jinhuang ointment. If there are many infected hair follicles and the pain is obvious, you should rest properly. If folliculitis recurs, you should check for systemic diseases. Vulvar Folliculitis Treatment 1. Eliminate the cause and keep the vulva clean and dry as much as possible. Avoid irritation and squeezing. 2. In the early stage of the disease, 0.02% potassium permanganate warm water sitz bath can be used twice a day. 3. If multiple small pustules are formed or merge into large pustules, systemic antibiotics can be used for treatment. 4. For mature pustules, puncture and drain to remove pus, rinse with normal saline or other external disinfectant, and then apply antibiotic ointment, sulfonamide ointment, or 2% iodine tincture on the periphery.5. If the pustules have not dried and scabbed 3 days after treatment, the cause should be investigated to determine whether there is concurrent Candida infection so that further treatment measures can be taken. 6. Special treatment for pubic folliculitis: Pubic folliculitis can be treated with Fulikang, which is usually widespread and severe. Moisture in the genitals makes it easier for bacteria to grow and spread. When using Fulikang, take an appropriate amount and apply evenly to the symptom area and surrounding radiated areas, and gently rub for 3-5 minutes until it is completely absorbed. Take "roxithromycin" orally 2-3 times a day during the treatment. Patients with vulvar folliculitis should pay special attention to daily cleanliness and hygiene, eat more vegetables and fruits, and foods rich in vitamins to improve the skin's own resistance. |
