Women who are pregnant need special care in their daily life, whether it is food, clothing, housing, transportation or other aspects. During pregnancy, the most feared thing is any problem. Ovarian chocolate cyst is a common ovarian disease. After this disease occurs, if it is not treated promptly, it will gradually grow larger and may rupture. How should a ruptured cyst of the 3rd month of pregnancy be treated? Let’s learn more about it. 1. Western Medicine 1. Surgical treatment 1. Surgery should be performed immediately after diagnosis, because the discharged cystic fluid may cause pelvic adhesions, infertility, or re-dissemination and implantation of ectopic endometrium. 2. For young infertile women, after suctioning and thoroughly flushing the cystic fluid that has overflowed into the pelvic cavity, a cystectomy should be performed to preserve normal ovarian tissue as much as possible. This will help maintain ovarian and endocrine functions and increase the chance of pregnancy in the future. 3. If both ovaries are affected, cystectomy should be performed as much as possible in principle. If the cyst is tightly adhered to the surrounding tissue and forced removal may easily damage the internal organs, anhydrous alcohol can be applied to the cyst cavity to cause necrosis of the epithelial layer in the cyst cavity to avoid recurrence in the future. Drug treatment is still advisable after surgery. 4. For those who are older and have children with normal contralateral ovary and uninvolved uterus, in order to avoid future recurrence, adnexectomy on the affected side may also be considered. 5. For patients with ruptured ovarian chocolate cyst, the abdominal cavity should be thoroughly cleaned during surgery, the lesion should be removed as much as possible, and adhesions should be loosened. Before closing the abdomen after surgery, 80,000 U of gentamicin, 5 mg of dexamethasone, 1000 U of hyaluronidase, and 250 ml of normal saline should be placed in the abdominal cavity to prevent postoperative adhesions. 6. After surgery, it is generally advisable to take medications to treat endometriosis to prevent lesions or cystic fluid that cannot be detected by the naked eye from contaminating the abdominal cavity and causing new dissemination and implantation of lesions. 2. Drug treatment
(1) Highly effective progestins: Treatment with high-effective progestins alone can inhibit endometrial hyperplasia, cause atrophy of the ectopic endometrium, and cause amenorrhea in patients. Generally, medroxyprogesterone (medroxyprogesterone), norgestrel (18-methylnorgestrel), etc. are used. If breakthrough vaginal bleeding occurs during treatment, a small amount of estrogen can be added, such as ethinyl estradiol 0.03 mg/d or conjugated estrogen (Premarin) 0.625 mg/d. The pregnancy rate after treatment is comparable to that of pseudopregnancy therapy, but the side effects are milder and most patients are able to persist in the treatment. (2) Gestrogens (18-methyl-norethindrone): a derivative of 19-nortestosterone, with a similar mechanism of action to danazol, but with a weaker androgenic effect. Because it has a long half-life in the body, it does not need to be taken every day. Usually start taking the medicine from the first day of menstruation, take 2.5 mg each time, twice a week. The pregnancy rate after treatment is similar to that of danazol, but the adverse reactions are milder, liver damage is less likely to occur, and the recurrence rate after discontinuation of the drug is also higher. It has been reported that the recurrence rate after one year of discontinuation of medication is 25%. 2. Traditional Chinese Medicine 1. Treatment with traditional Chinese medicine: Phellodendron chinense, Rhizoma Dictamni, frankincense, myrrh, and Vaccinium macrostemon, etc. 2.
