For young women in the puberty process, physical therapy is usually effective, and conservative treatment is the first choice.

2. Surgery to preserve the mammary ducts for the treatment of inverted nipples

Suitable for patients with mild, moderate and severe degrees of nipple inversion.

First, use No. 1 silk thread to suture two stitches above and below the nipple to pull the inverted nipple. The nipple is incised transversely along the direction of nipple inversion, the mammary ducts are separated within the nipple incision, and the shortened fiber bundles between the ducts are cut off until the nipple no longer retracts after the traction line is released. After the fiber bundles are released, the nipple still retracts inward, and it is often necessary to cut off part of the middle part of the mammary duct until the inverted nipple is corrected. During the operation, retaining several obvious mammary ducts can lay the foundation for future breastfeeding. Complete release of shortened fiber bundles is an essential component of any surgical approach. If the release is incomplete, recurrence will often occur after surgery. After releasing the fiber bundle, there is a lack of tissue filling under the nipple, leaving a dead space, which is one of the reasons for the recurrence of infection after nipple inversion surgery. The following measures should be taken according to the degree of tissue defect:

① If the dead space is small, directly pull it together and suture it, and avoid occluding the mammary duct during suturing.

② When designing a breast tissue flap to fill the dead space at the base of the nipple, attention should be paid to maintaining the blood supply of the breast tissue flap.

③ Design dermal tissue flaps at both ends of the incision to fill the defect. After suturing the inside of the nipple, the nipple is usually narrow at the top and wide at the bottom. Performing Z-plasty at both ends of the incision can narrow the nipple neck and improve the shape of the nipple, further preventing the recurrence of nipple retraction. When bandaging inverted nipple surgery, be careful to prevent the nipple from being compressed. Make a hole in the middle of the dressing or sponge and place the nipple in it. Use traction thread to pull the nipple and fix it for 2 weeks. Remove the remaining sutures 1 week after the surgery and do not wear a bra within one month.

3. Mammary duct cutting surgery

Broadbent surgery can be used for women who have given birth and do not plan to breastfeed in the future, or for patients with recurrent local inflammation and severe scar traction and deformity.

The treatment before and after nipple inversion surgery is the same as the above method. During the operation, the scar at the base of the nipple is removed, and any inflammatory mass is also removed. Completely cut off the breast duct, fully release the inverted nipple, and design a tissue flap to fill the tissue defect at the root of the nipple. Preventing dead space formation is the key to successful nipple inverted surgery.

If women find that their nipples are inverted (retracted nipples), it is best to seek correction in a timely manner. For mild inverted nipples, you can use a corrector to correct them first.
