Cervical malignant melanoma is a malignant tumor originating from melanocytes in the basal layer of the epithelium. Malignant melanoma of the female genital tract accounts for 3% of all malignant melanomas in women. It mainly occurs in the vulva and vagina. Malignant melanoma occurring in the cervix is ​​a clinically extremely rare gynecological tumor.

Differential Diagnosis

The diagnosis of oligomelanotic or amelanotic malignant melanoma is more difficult because its tissue morphology is similar to that of many tumors. When making a pathological diagnosis, it is necessary to differentiate it from cancer (squamous cell carcinoma, adenocarcinoma, undifferentiated carcinoma), sarcoma, carcinosarcoma, lymphoma and choriocarcinoma. In addition to observation of HE sections under a microscope, immunohistochemical staining is often used to assist in the diagnosis. HMB45 positivity has a higher specificity, S-100 and Vimentin have high sensitivity, and NSE is also helpful for diagnosis. Commonly used methods include melanin granule staining (Fontant staining), S-100, and HMB45 positive tests, and CK negative tests to confirm the diagnosis. Malignant melanoma of the cervix must also be differentiated from cervical blue nevus, which is flat and slightly higher than the mucosa to the naked eye, blue to black, with unclear boundaries, more common in the posterior lip, and no active cell proliferation at the junction of the epidermis and dermis.
