Lichen sclerosus malnutrition begins as small white papules with irregular edges and a flat surface, which gradually merge into white spots, hence the name lichen, and further merge into large white spots. After the lesion has fully developed, the skin becomes wrinkled, becomes thin, hard, and brittle like brown paper, and loses its elasticity, gradually leading to atrophy of the entire vulva. Microscopically, there is epithelial atrophy, hyperkeratosis, hyalinization of the collagen fibers in the dermis, and chronic inflammatory cell infiltration in the mid-dermis.

3. Mixed vulvar dystrophy is a mixture of the above two types. On the basis of the atrophic type, there is local epithelial hyperplasia and hypertrophy. Microscopically, hyperplastic islands can be seen in the atrophic lesions.

4. Vulvar leukoplakia with atypical hyperplasia. All the above-mentioned vulvar leukoplakia may be accompanied by atypical hyperplasia, but hyperplastic vulvar leukoplakia is more common. Microscopically, the spinous cell layer is arranged without layers, the cells are of different sizes, and some have increased divisions. It can be divided into three levels: mild, moderate and severe according to the severity. Atypical hyperplasia is mild when it is limited to the deep epidermis, moderate when it involves the middle epidermis, and severe when it involves the entire layer. Patients should be very alert to the dangers of vulvar leukoplakia and actively seek treatment.
