Precautions

1. When performing extrafascial total hysterectomy to remove 1.5 cm of parametrial tissue, attention should be paid to the course of the ureter. The ureter must be freed 3 cm upward from the intersection of the ureter and the uterine blood vessels, and the blood vessels must be cut and ligated on the outside of the ureter. After the ureter is pushed open, the parametrial tissue should be clamped and cut to avoid damaging the ureter.

2. It is not necessary to open the ureteral tunnel.

3. Circumcise the vaginal wall 1 to 1.5 cm away from the vaginal vault, remove the uterus, and suture the vaginal ends fully. It is not necessary to leave a drainage tube.

4. Women with normal ovaries before menopause should retain their ovaries.

5. The urinary catheter should be retained for 72 hours after surgery.

6. Give antibiotics after surgery to prevent infection.

7. Get up and move around appropriately early after surgery.

8. No sexual intercourse for 3 months after surgery.

9. Other precautions are the same as those for total abdominal hysterectomy and subradical hysterectomy.

Contraindications

1. Patients with cervical cancer stage Ia2 and above.

2. Patients with poor general condition or combined diseases cannot tolerate surgery above extrafascial hysterectomy.

The excision range is the entire uterus, 1 to 1.5 cm outside the lesion, and 1 to 1.5 cm of the vaginal wall.
