1. Tumor rupture

Traumatic rupture is often caused by severe abdominal blow, childbirth, sexual intercourse, gynecological examination and puncture, while spontaneous rupture is often caused by rapid tumor growth, most of which are invasive tumor growth that penetrates the cyst wall. The severity of the symptoms depends on the size of the rupture and the nature and amount of cystic fluid that flows into the abdominal cavity. When a small cyst or simple serous cystadenoma ruptures, the patient only feels mild abdominal pain. When a large cyst or mature teratoma ruptures, it often causes severe abdominal pain, nausea and vomiting, and sometimes leads to internal bleeding, peritonitis and shock.

2. Infection

It is less common and is usually caused by tumor twisting or rupture, or it may spread from infection in adjacent organs such as appendix abscess. Clinical manifestations include fever, abdominal pain, lumps and tenderness, abdominal muscle tension and increased white blood cell count. Treatment begins with antibiotics, followed by surgery to remove the tumor. If the infection cannot be controlled in a short period of time, surgery should be performed immediately.

3. Pedicle torsion

It is more common and is one of the gynecological acute abdomen. About 10% of ovarian tumors are complicated by pedicle torsion. It is more common in cystic tumors with long pedicles, medium size, high mobility, and center of gravity biased to one side. It often occurs during sudden changes in body position, early pregnancy, or after delivery. The pedicle of ovarian tumors is composed of the pelvic infundibulocele ligament, the ovarian proper ligament and the fallopian tube. After acute torsion occurs, venous return is blocked, the tumor becomes highly congested or blood vessels rupture, causing the tumor to rapidly increase in size and intratumoral bleeding. Finally, arterial blood flow is blocked, the tumor undergoes necrosis and turns purple-black, making it prone to rupture and secondary infection.

4. Malignant transformation

Malignant transformation of benign ovarian tumors often occurs in older women, especially after menopause. The tumor grows rapidly in a short period of time, and the patient feels abdominal distension and loss of appetite. Examination shows that the tumor volume has increased significantly, is fixed, and often has ascites. Those suspected of malignant transformation should be treated promptly.
