Epidemiology丨What is the most common type of HPV in China? Why is it said that posture determines destiny?

China is a region with a high burden of human papillomavirus (HPV)-related cancers, and the incidence and mortality of cervical cancer rank second in the world. According to previous data, my country has a bimodal distribution of cervical cancer (see figure below).

2012 Study

What’s interesting about the 2023 study is that the two peak age groups in the 2012 study were 15-24 and 40-44, while the study of people who had not received the HPV vaccine, published in November 2023, was in the 10-19 and 60-69 age groups (below).

In addition, my country's HPV has these interesting characteristics:

● Types 6 and 11 covered by the quadrivalent and nine-valent HPV vaccines can cause genital warts, but in the unvaccinated population, the low-risk types rank sixth and eighth, respectively (see figure below);

● The three types of HPV 16, 18 and 33 covered by the vaccine (both the bivalent and quadrivalent vaccines show some cross-protection, with the bivalent vaccine having more obvious cross-protection, so 33 is also covered) are on a downward trend; the four types of HPV 52, 53, 59 and 61 are on an upward trend (see the figure below);

● Although the main high-risk types globally are HPV16, 18, 52, 31 and 58, the top five types in my country are HPV 52, 58, 53, 16 and 39 (Figure 3, 53 and 39 are not even covered by the 9-valent HPV vaccine, and type 18 is left behind);

●I have always said that "posture determines fate" because HPV is also related to oral infection and head and neck cancer. Taking the data from Hebei Province as an example, the overall oral HPV infection rate is 4.08%, the high-risk infection rate is 1.92%, and the low-risk HPV infection rate is 2.16% (see the figure below).

Finally, here’s a little-known fact: the incidence rates of ovarian cancer, cervical cancer, exogenous cancer, and vaginal cancer among men in my country are much lower than those among women (see figure below). Isn’t it amazing?
