Therefore, it is particularly important for patients with chronic kidney disease to master the correct blood pressure measurement method and closely monitor blood pressure changes. Methods for measuring blood pressure include office blood pressure monitoring, home blood pressure monitoring and 24-hour dynamic blood pressure monitoring.

0 1 Blood pressure monitoring in clinic

Blood pressure monitoring in the clinic usually uses a mercury sphygmomanometer. The operation of the mercury sphygmomanometer is relatively complicated, and the user needs to have certain medical expertise, and the patient and family members can also master it after training.

The following points should be noted during operation: (1) The subject should sit and rest quietly for 5 minutes before starting the measurement. (2) When measuring the upper arm blood pressure in the sitting position, the upper arm should be placed at the level of the heart. (3) Inflate the balloon quickly to a pressure just after the radial artery pulsation disappears, then increase it by 30 mmHg and then slowly deflate it. The reading when the "dongdong" sound is just heard is the systolic pressure, and the reading when the "dongdong" sound disappears or suddenly weakens is the diastolic pressure. After obtaining the diastolic pressure reading, deflate it quickly to zero. (4) Measure three times in a row, with at least 1 to 2 minutes between each measurement, and record the average of the three readings. If the difference between the two measurement results is large (more than 5 mmHg), measure again.

Home blood pressure monitoring usually uses an electronic blood pressure monitor. The operation of the electronic blood pressure monitor is relatively simple and easier to master. However, some patients have reported that the values ​​measured by the electronic blood pressure monitor fluctuate greatly and have poor reliability.

Therefore, when using an electronic blood pressure monitor, you should try to avoid errors caused by improper operation methods, as follows: (1) When using an arm cuff blood pressure monitor, you should take off your sleeves and there should not be too much clothing between the cuff and your arm. After tying the cuff, it is best to leave a gap that can accommodate a finger. (2) When using a wrist cuff blood pressure monitor to measure blood pressure in a sitting position, the wrist should be level with the heart. (3) Do not move or talk during measurement. Measure 2 to 3 times each time and take the average value.

In addition, when monitoring your blood pressure at home, be sure to record in detail the date, time, and all blood pressure readings of each blood pressure measurement, and provide the doctor with complete blood pressure records during follow-up visits.

0 324-hour dynamic blood pressure monitoring

24-hour dynamic blood pressure monitoring uses a dynamic recorder to measure a person's blood pressure at certain intervals for 24 hours a day and night. The recorder box is carried on the back, so it is commonly known as the "back box". Dynamic monitoring helps to understand the characteristics of blood pressure fluctuations and the severity of the disease, and can also identify "white coat hypertension". For example: Mr. Li was diagnosed with hypertension two years ago, but he has never taken medication. This year, the results of the "back box" showed that his blood pressure was normal. After taking it off, the doctor measured that his blood pressure was higher again, but Mr. Li went home and measured it himself many times and it was normal. Mr. Li also muttered: Could it be that I didn't measure it accurately? In fact, the patient's situation is a typical "white coat hypertension". It may be because when seeing doctors and nurses in white coats, they can't help but feel nervous and anxious, causing increased sympathetic nerve activity. "White coat hypertension" is only a temporary reactive blood pressure increase, and its actual blood pressure is at a normal level, so it belongs to "pseudo-hypertension", also known as "clinic hypertension". Through 24-hour dynamic blood pressure monitoring, the patient carries the blood pressure device himself without the presence of medical staff, which can distinguish "white coat hypertension" from true hypertension.
