1. From the perspective of the fetus, episiotomy will be done when there is intrauterine rupture or premature birth to relieve pressure on the fetus and to complete delivery as soon as possible.

2. From the mother's perspective, sometimes a midwifery vacuum extraction or antenatal midwifery is needed, and if the elasticity of the perineum is not very good, an episiotomy may be performed to prevent more serious tearing.

3. The episiotomy can quickly reduce the friction resistance of the soft birth canal, accelerate the delivery rate, and prevent perineal tearing in pregnant women and protect the pelvic floor muscles. Accidental tears that are not controlled by human factors, especially lacerations around the 6 o'clock position, can easily lead to lacerations of the external anal sphincter, and in severe cases can cause fecal incontinence - the risk outweighs the gain. And surgical incisions are much easier to repair and heal much better.
