1. Ovulation abdominal pain

During the ovulation period of adolescent girls, the follicle ruptures and the egg fluid may have a certain stimulating effect on the retroperitoneum, so they sometimes experience mild abdominal pain that alternates between upper and lower parts once a month. This is physiological, and the main manifestation is mostly unilateral lower abdominal dull pain, dull pain or heaviness-like pain. Some girls are also accompanied by a small amount of vaginal bleeding, that is, ovulation bleeding, which usually disappears on its own after one or two days, and generally does not exceed seven days. This type of abdominal pain usually does not have any pathological changes, and gynecological examination is completely normal. It belongs to physiological abdominal pain and generally does not need to be treated.

2. Ectopic pregnancy

Ectopic pregnancy refers to the implantation of the embryo in the gestational sac outside the uterus. Among ectopic pregnancies, about 95% occur in the bilateral fallopian tubes. Abnormal development of the bilateral fallopian tubes, abnormal function of the bilateral fallopian tubes, compression of tumors, sinking of contraceptive rings, etc. may all lead to ectopic pregnancy. Lower abdominal pain is one of the common symptoms of ectopic pregnancy. In addition, ectopic pregnancy is also accompanied by symptoms such as amenorrhea and irregular vaginal bleeding.

3. Menstrual pain

Menstrual pain is divided into two types: primary and secondary menstrual pain. Primary dysmenorrhea is more common in adolescent girls. It is related to the body's prostacyclin level, cold, and psychological disorders without organic diseases. It can usually be cured with aging or after marriage and childbirth. The common causes of secondary dysmenorrhea are endometriosis and adenomyosis. The main symptoms are gradually increasing pain, which often occurs regularly, or dull pain in the lower abdomen during non-menstrual periods that worsens before and after menstruation. In addition, it is often accompanied by infertility and menstrual disorders, which usually require medication.

4. Ovarian Corpus Luteum Dehiscence

It is caused by the rupture of multiple progesterone capillaries, accompanied by sudden onset of lower abdominal or unilateral abdominal pain, and in severe cases, shock may occur. Most cases of ovarian corpus luteum dehiscence occur between the 20th and 26th days of the menstrual cycle, and lower abdominal pain may occur, which may vary in severity. Before the corpus luteum of the ovary breaks apart, there is a process of uterine ovarian hematoma and swelling. When the uterus and ovary are affected by external force or indirect external force, especially the pre-menstrual hematoma, such as caused by straining during defecation, trauma, sexual intercourse, and strenuous activity. It is best to go to the hospital for treatment immediately after the onset of ovarian corpus luteum rupture. Do not use painkillers indiscriminately to avoid masking the symptoms and affecting normal diagnosis and treatment.

5. Ovarian cyst pedicle torsion or rupture

This is because the cyst has a long pedicle, large volume, no adhesions to the surrounding area, and high activity, which can cause distortion when affected by gastrointestinal function or posture changes. When the pedicle of the uterus and ovary tumor is twisted, pain suddenly occurs on one side of the lower abdomen, which is continuous pain, often accompanied by nausea and vomiting; there may be obvious tenderness and muscle tension in the abdomen; digital rectal examination can reveal swollen and dull pain in the appendages. Ovarian cysts may also rupture, and the contents of the ruptured cysts may irritate the retroperitoneum and cause severe pain. B-ultrasound examination can make the diagnosis. Generally, critical surgery is required.

6. Pregnancy-related illnesses

In recent years, case reports of teenage girls becoming pregnant are quite common. Abdominal pain caused by pregnancy-related diseases can be seen in threatened abortion, ectopic pregnancy, etc. Taking ectopic pregnancy as an example, there may be dull pain or swelling in the adnexal area of ​​the affected limb. When the ectopic pregnancy leads to miscarriage or rupture, it may cause rupture of abdominal capillaries. The pain symptoms vary depending on the amount and rate of bleeding. If it is a miscarriage due to fallopian tube pregnancy, abdominal pain is often limited to one side of the lower abdomen, and blood accumulates in the uterine duodenal recess, which can cause anal pain. When the fallopian tube pregnancy ruptures, there is a lot of bleeding and the speed is faster. The blood quickly spreads to the whole abdomen, causing whole abdominal pain. In addition, some pregnant girls have abortions without authorization, which is not only easy to cause infections such as pelvic inflammatory disease or trauma to the pelvic organs and reproductive system, but also seriously endangers personal safety.

7. Acute appendicitis

It is a common lower abdominal pain in young girls. It is a surgical disease, but it needs to be distinguished from gynecological diseases. About 70% to 80% of patients initially feel pain in the upper abdomen or around the navel, which migrates to the right lower abdomen after a few hours. The abdominal pain of simple appendicitis is localized in the right lower abdomen and may be continuous or paroxysmal. The abdominal pain of purulent appendicitis worsens, often presenting as intermittent severe pain or distending pain. Necrotic appendicitis initially presents with persistent throbbing pain, with the pain expanding in range and becoming more severe. In the early stages, there is often mild nausea and vomiting, often accompanied by loss of appetite and constipation. If the inflammation is severe, abdominal distension may occur. The presence of right lower abdominal pain due to metastatic cancer is a characteristic of appendicitis. This disease generally requires surgical removal for treatment.

8. Psychogenic abdominal pain

If there is a girl with school phobia, she will be worried about studying and taking exams. If the patient is forced to go to school, she will develop depression and anxiety-related physical discomfort, such as pale face, rapid heart rate, shortness of breath, unbearable abdominal pain, nausea, headache, dizziness, etc. Some patients seek medical treatment for severe abdominal pain many times, but various test results are normal. Once the patient is temporarily discharged from school, the depression and discomfort symptoms will be quickly alleviated, and the abdominal pain will disappear immediately. These patients require psychological therapy.
