Clear linear tenderness is mostly caused by fractures, and the fracture line can be determined along the direction of the tenderness; conducted percussion pain is lighter than lumbar fracture, especially when examined in a standing position.

2. Pathogenesis

The mechanism of sacral fracture associated with pelvic fracture is the same as that of pelvic fracture, which is mostly caused by simultaneous front-to-back compression of the pelvis. Please refer to the pelvic fracture section. Only single sacral fractures are discussed here.

1. Direct violence

The most common cause is landing on the sacrum when falling, sliding or rolling from a height; the second most common cause is being hit by a heavy object or a direct impact from a vehicle or other object.

2. Indirect violence

It is more common for violence to be transmitted upward from the bottom (distal end of the sacrum and coccyx), while the chance of violence being transmitted from top to bottom is very small; avulsion fractures may also be caused by ligament traction.

Among the most common combined injuries, most are caused by pelvic fractures, most of which are caused by direct violence; while the complications of sacral fractures mainly involve the rectum and anus.

The differential diagnosis should be made with the following symptoms:

1. Sacral cyst

Bone cyst is a tumor-like lesion of bone, also known as solitary bone cyst or simple bone cyst. The cyst wall is a fibrous capsule and the cyst contains yellow or brown liquid.

2. Sacral fissure

Sacral cleft is a skeletal developmental abnormality or congenital malformation of the lumbosacral region caused by incomplete closure of the spinal canal during the embryonic period. During the development of the first and second sacral vertebrae, the initial osteogenesis of the vertebral arches on both sides is incomplete, the vertebral plates are not fused, but there is no spinal cord herniation. This is medically known as occult sacral bifida, also known as occult spina bifida.
