This is used to illustrate the average direction and intensity of the ventricular potential during the total depolarization process.

There are many factors that affect the direction of the cardiac axis, mainly the following aspects:

② Age: The cardiac axis of infants and young children tends to be right-biased, while that of the elderly tends to be left-biased.

④ The weight ratio of the left and right ventricles of the heart: the cardiac axis of patients with right ventricular hypertrophy is often right-deflected, while that of patients with left ventricular hypertrophy is often left-deflected.

⑤ The functional state of the electrical impulse conduction pathway within the ventricle, namely the bundle branch: when the left anterior branch of the electrical impulse is blocked, the cardiac axis may deviate to the left, while when the left posterior branch is blocked, the cardiac axis may deviate to the right.

Why does left axis deviation occur in sinus rhythm?

The normal range of the cardiac axis is between -30° and +90°; an axis between -30 and -90° is considered left axis deviation. Left axis deviation in sinus rhythm is a physiological symptom of life. In normal people, left ventricular hypertrophy and left posterior fascicular block can cause left deviation of the cardiac axis. People with a slender body shape will generally also have left deviation. For example, when you are playing in an amusement park, riding a roller coaster or an airplane and feel like you can't breathe, this is what is commonly known as "sinus rhythm." "Left axis deviation in sinus rhythm" is a normal physiological phenomenon, not a serious disease. "Left axis deviation of sinus rhythm" is most common in children and adolescents, but is also not uncommon in adults.

The hazards of left axis deviation in sinus rhythm:

The left axis deviation of sinus rhythm mainly refers to the indirect response indicator of the electrocardiogram. We cannot draw conclusions about the overall health indicators based on this indicator alone. If the ECG diagnoses left axis deviation in sinus rhythm, the cause of the symptoms needs to be sought, especially if there is no heart or other chest organ disease, such as coronary heart disease, cardiomyopathy, myocarditis, some congenital heart diseases, and degenerative diseases, because these diseases may damage the left anterior fascicle and cause left axis deviation.

Patients with diseases such as hypertension and aortic valve abnormalities may suffer left axis deviation due to left ventricular hypertrophy. Patients with chronic bronchitis accompanied by emphysema and cor pulmonale may experience false left axis deviation, etc. Once a disease is diagnosed, treatment must be given specifically for that disease, but there is usually no special treatment for left axis deviation itself.

If there is only left axis deviation and no cardiothoracic disease has been diagnosed, and no other signs of further intraventricular block occur during follow-up observation, it can be considered that this left axis deviation has no important pathological significance. At this time, it is advisable to pay attention to finding out whether there are other reasons mentioned above that may cause left axis deviation of the heart.
