1. Causes

1. Inflammatory vaginitis, cervicitis, endometritis, pelvic inflammatory disease, pelvic effusion, gonorrhea, condyloma acuminatum, genital herpes, etc. can all cause abnormal leucorrhea.

2. Tumors such as uterine fibroids, cervical cancer, endometrial cancer, vaginal cancer, pelvic tumors, fallopian tube and ovarian tumors, etc.

3. Foreign bodies, such as intrauterine contraceptive rings, vaginal pessaries, or foreign bodies left in the vagina after delivery or surgery, as well as foreign bodies accidentally entering the vagina, may irritate the vagina and induce inflammation, causing a large amount of yellow purulent leucorrhea with a fishy smell, or even blood in the leucorrhea.

4. Endocrine ovarian dysfunction and insufficient secretion of sex hormones can cause too little leucorrhea and vaginal dryness; taking estrogen drugs can cause leucorrhea to become sticky and profuse.

5. Other vesicovaginal fistulas and rectovaginal fistulas are caused by excrement irritating the vagina, resulting in increased vaginal secretions. A highly retroflexed uterus, chronic systemic diseases such as heart failure, diabetes, anemia, tuberculosis, and mental stimulation can all cause changes in leucorrhea.

3. Typical symptoms

1. Cheese-like or tofu-like leucorrhea: These are typical symptoms of candidal vaginitis, often accompanied by severe vulvar itching.

2. Yellow watery leucorrhea: yellow or yellow-green, sticky or foamy, with a foul odor. Trichomonas vaginitis is common, with vulvar pruritus.

3. Fishy odor leucorrhea: a characteristic of bacterial vaginosis.

4. Urinary dysfunction with purulent leucorrhea: typical symptoms of gonococcal infection. Cervicitis caused by chlamydia, with increased white serous discharge.

5. Purulent leucorrhea: consider acute pelvic infection, cervical cancer, vaginal cancer with infection, uterine cavity pyometra, foreign body in the vagina, etc.

6. Watery discharge: swill-like, with a foul odor, seen in cervical cancer, vaginal cancer, or submucosal fibroid infection. Paroxysmal vaginal discharge: hydrosalpinx or fallopian tube cancer needs to be excluded.

7. Bloody leucorrhea: It may be cervical polyps or submucosal fibroids; at the same time, be alert to malignant tumors such as cervical cancer.

3. Treatment Methods

Drug treatment:

1. Trichomonas vaginitis. Trichomonas exist in the vaginal folds and glands of the cervix, as well as in the urethra and intestines. Therefore, it should be treated both internally and externally. ① Oral anti-trichomoniasis drugs. After taking the medicine, test for Trichomonas. If the result is negative, the patient should continue treatment for one course after the next menstruation to consolidate the therapeutic effect. ②Simultaneous local treatment. ③ Husband and wife govern together.

2. For candidal vaginitis, you can use soda solution to rinse the vulva, vagina or take a sitz bath. After gently drying, place a nystatin suppository deep in the vagina, or apply nystatin cream on the vaginal wall, once a night or once in the morning and evening for a total of 10 to 14 days.

3. Metronidazole can be taken orally as an optional drug for nonspecific vaginitis. It can also be treated with local medication.

4. Senile vaginitis can be treated with local vaginal medication. Increase vaginal acidity. If necessary, estrogen therapy should be used as an adjunct.

Other treatments:

Change the acidity and alkalinity of the vagina: It is advisable to use a feminine care solution with a weak acid formula of pH ≤ 4. In addition to being suitable for daily cleaning and maintenance, the use of a feminine care solution with a weak acid formula during treatment will have an inhibitory effect on the growth and reproduction of mold. When going to the toilet, wipe the vulva with feminine hygiene wipes and keep it dry to inhibit the growth of Candida.
