1. Inspection

1. Menstruation

Excessive menstrual flow is mostly due to blood heat or qi deficiency; insufficient menstrual flow is mostly due to blood deficiency, kidney deficiency or blood stasis caused by cold; alternating more and less menstrual flow is mostly due to qi stagnation and kidney deficiency. Purple or bright red menstrual blood indicates heat in the blood; light red menstrual blood indicates qi or blood deficiency; dark purple menstrual blood indicates blood stasis. Thick and sticky menstrual blood usually indicates blood stasis and heat; thin and thin menstrual blood usually indicates deficiency and cold; dark purple blood clots usually indicate blood stasis.

2. Expect leucorrhea

Excessive leucorrhea is pathological, and may be caused by severe dampness and heat, or spleen or kidney deficiency, and must be carefully distinguished in clinical practice. If the leucorrhea is white, it is mostly due to spleen deficiency or kidney deficiency; if the leucorrhea is yellow, it is mostly due to damp heat or dampness toxins; if the leucorrhea is red or a mixture of red and white, it is mostly due to blood heat or evil toxins; if the leucorrhea is clear and thin, it is mostly due to spleen deficiency or kidney deficiency; if the leucorrhea is thick and sticky, it is mostly due to accumulation of damp heat.

3. Lochia

If the amount of bad fluid is large, the color is light, and the texture is thin, it is mostly due to Qi deficiency; if it is bright red or purple-red, and thick and sticky, it is mostly due to blood heat; if it is purple-black with lumps, it is mostly due to blood stasis.

1. Listen to the sound

Listening to the patient's voice, breathing, belching, sighing, spitting, wheezing, coughing and other sounds can help determine which organs the disease is in and whether it is a deficiency or a real condition. If the voice is low, it is mostly due to insufficient internal energy; if one is unhappy and speaks little, but wants to sigh from time to time, it is mostly due to liver qi stagnation; if the voice is high and the breath is rough, or even the speech is incoherent, it is mostly due to excess syndrome or heat syndrome; if there is frequent belching, or nausea and vomiting, it is mostly due to stomach qi reflux and spleen and stomach disharmony; if there is wheezing, coughing and shortness of breath, it is mostly due to fluid stagnation in the heart, or lung qi failure.

2. Smell

Understand the patient's body and the smell of the ward to distinguish between yin and yang, cold and heat. In gynecology, the main thing is to understand the odors of menstruation, leucorrhea, lochia, etc. If the smell is fishy, ​​it is mostly due to cold and dampness; if the smell is foul, it is mostly due to blood heat or accumulation of damp heat; if the smell is foul, it is mostly due to excessive evil toxins, or stasis and pus, which is a dangerous clinical condition.

3. Consultation

1. Age

Women of different ages have different physiological differences and thus different emphases in their pathology, so the treatments also have different focuses. Generally speaking, adolescence is prone to menstrual problems due to insufficient kidney qi. Middle-aged women suffer from blood damage due to pregnancy, childbirth and breastfeeding, and their liver and kidneys are malnourished, which often leads to irregular menstruation and various pre- and post-natal diseases. Elderly women with spleen and kidney deficiency are prone to various symptoms before and after menstruation, malignant tumors, etc.

2. Chief complaint

The chief complaint should include two elements, namely the nature of the main symptoms and the time of occurrence. The chief complaint must be clearly stated during medical consultation, and the text should be concise and accurate when written down. The chief complaint provides clues for other gynecological consultations and is of great value in the diagnosis of the disease.

3. History of present illness

It includes the cause or trigger of the disease, the severity of onset, the initial symptoms, the treatment process and effects, the current symptoms, etc.

4. Menstrual history

Understand the age of menarche, the date of the last menstrual period, the date of the last menstrual period before, the menstrual cycle, the number of days of menstruation, changes in menstrual volume, color and quality, symptoms before and after menstruation, and the current situation or before and after menstruation. The numbers 28 to 30 below the 4th to 5th horizontal line on the 14th horizontal line are commonly used to indicate the age of menarche, menstrual period, and cycle. Early menstruation is mostly due to blood heat or qi deficiency; late menstruation is mostly due to blood deficiency or cold stagnation; early or late menstruation is mostly due to liver depression or kidney deficiency.
