During sexual intercourse, the Bartholin's glands secrete some fluid to act as a lubricant. But if the hole of the Bartholin gland is blocked, a Bartholin gland cyst will form. If it is further infected by bacteria, the Bartholin gland will become inflamed, red, swollen, hot and painful, which is called a Bartholin gland abscess.

Clinical manifestations and diagnosis

Acute Bartholinitis first invades the glandular duct, presenting as acute suppurative inflammatory changes with local redness, swelling, heat, and pain. Sometimes there are systemic symptoms such as feeling of heaviness, difficulty in urination and defecation, increased body temperature, and increased white blood cell count. The opening of the gland duct is often blocked due to swelling or coagulation of exudate, and the pus cannot flow out, forming an abscess, which is called Bartholin's gland abscess. There may be a local sense of fluctuation and enlarged inguinal lymph nodes. When the pressure inside the abscess cavity increases, it may rupture on its own. If the wound is large and the drainage is smooth, the inflammation can be eliminated and healed quickly. If the wound is small and the drainage is not smooth, the disease may recur, often making the patient restless when walking or lying down.

When the acute inflammation disappears, the gland duct opening is blocked, the glandular secretion fluid cannot be discharged or the pus gradually turns into mucus to form a cyst, which is called Bartholin's gland cyst.

Based on medical history and subjective symptoms. If there is a red, swollen, tender lump or a sense of fluctuation in the Bartholin's gland area at the vaginal opening, the diagnosis can be confirmed.

Pathogens

Staphylococcus, Escherichia coli, Streptococcus and Enterococcus, etc., are often mixed infections.

Prevention and treatment

Women, especially those who have sex, should pay attention to personal hygiene, and their sexual partners should also pay attention to hygiene. In addition, physical exercise should be strengthened to improve physical fitness.

While using antibiotics for treatment, you can also choose to take a sitz bath with Chinese herbal decoctions that can clear away heat, detoxify, reduce swelling and disperse nodules under the guidance of a doctor. Chinese herbal medicines include Forsythia suspensa, Honeysuckle, Wild Chrysanthemum, Viola yedoensis, Phellodendron chinense, Scrophularia ningpoensis, Isatis indigotica, etc. Sit in the bath for 20 minutes each time when the medicine is cool, 1 to 2 times a day. After an abscess is formed, incision, drainage and ostomy should be performed promptly. Simple drainage can only temporarily relieve symptoms, and Bartholin gland abscess or cyst may still form after the incision is closed. If there is a small wound and the pus cannot be discharged smoothly, the wound should be enlarged and drained.
