After giving birth, the mother must have a good rest, because giving birth not only consumes a lot of physical energy, but also damages the vitality of the body. During the confinement period, there are many things to pay attention to. It is best to ask experienced elders such as your mother-in-law or your own mother to help you through the confinement period. So the question is, what is the disease that causes pain in the lower body when standing after childbirth?

2% to 9% of postpartum women develop urinary tract infection, the most common types of which are cystitis and pyelonephritis. The most common pathogens of urinary tract infections are intestinal Gram-negative bacilli, of which Escherichia coli accounts for more than 70%. Urinary tract infections are usually caused by ascending infections, that is, bacteria ascend along the urethra to the bladder, ureters and kidneys to cause infection.

1. Causes and related diseases

During pregnancy: ① the ureter, renal pelvis and renal calyx dilate; ② the incidence of vesicoureteral reflux increases, and reflux can allow bacteria in the bladder to ascend with urine; ③ the carbohydrate content in urine during pregnancy increases, becoming a good culture medium for bacteria, which is conducive to bacterial growth; ④ in the late pregnancy, the fetal head compresses the bladder and the lower end of the ureter, resulting in difficulty in urination. Therefore, pregnant women are susceptible to urinary tract infections. In addition, the urethra may be damaged after delivery, and catheters are inserted multiple times during delivery, causing congestion in the pelvic urethra. The resistance of women in the puerperium is reduced, which can easily lead to bacterial invasion, so they are more susceptible to infection. The main pathogens are Escherichia coli, followed by streptococci and staphylococci, and mixed infections are often seen clinically.

2. Differential Diagnosis

Pyelonephritis: Typical acute pyelonephritis is easy to diagnose based on medical history, physical signs and urine tests. For patients with chronic pyelonephritis, if they develop fever or upper respiratory tract infection after delivery, they should be alert to the possibility of acute attacks of pyelonephritis. Unilateral pyelonephritis, especially on the right side, often presents with right lower abdominal pain and vomiting, so it must be differentiated from appendicitis.

Cystitis: Since the clinical manifestations of cystitis in the postpartum period are basically the same as those in non-pregnancy period, and it is often secondary to postpartum urinary retention, it is not difficult to diagnose.

3. Treatment and Care

Ensure adequate rest and nutrition, empty the bladder in time, and give sensitive antibiotic treatment.

1. General care

During the acute infection period, you should stay in bed. Encourage patients to drink plenty of water, 4000 mL per day, to flush the bladder.

Prenatal care should assess for symptoms of urinary tract infection.

Try to avoid catheterization during labor and delivery.

Empty your bladder at least once every 2 to 4 hours after delivery.

Clean the perineum promptly after urination and defecation, and replace the disinfected perineal pad.

2. Diet care

Provide high-calorie, high-protein, nutritious, easily digestible and less irritating food to enhance the body's resistance.

3. Symptom care

If the patient develops fever, give symptomatic treatment to reduce the temperature. When the patient experiences discomfort such as frequent urination, urgency, and pain when urinating, antispasmodics and analgesics should be given appropriately to relieve the discomfort. And drink plenty of water to flush the bladder.
