For women, once abdominal pain occurs, in addition to considering whether they have internal or surgical diseases, factors unique to women should also be taken into account.

Dysmenorrhea

Because everyone's subjective perception and tolerance to pain is different. There is a lack of objective measurement standards for pain in medicine, so gynecologists use "whether there is vomiting or fainting during menstruation, whether one can continue to work, and whether one needs to stay in bed to rest" as the measurement standard for the severity of dysmenorrhea. Dysmenorrhea without uterine and pelvic lesions is primary dysmenorrhea, and the spontaneous healing rate of this type of dysmenorrhea may increase with age. Because the occurrence of primary dysmenorrhea is sometimes related to endocrine factors, taking contraceptives after marriage to inhibit ovulation can make dysmenorrhea disappear. Some people have a narrow cervical canal, which affects the outflow of menstrual blood and causes dysmenorrhea. After delivery, the cervical canal relaxes and menstrual blood flows unobstructed, so there will be no pain. As for dysmenorrhea caused by lesions of the reproductive organs, the primary disease must be treated.

Ovulation pain

During ovulation, a small amount of fluid in the egg cell overflows into the abdominal cavity and stimulates the peritoneum. Causes abdominal pain. The ovaries on both sides ovulate alternately, and abdominal pain occurs alternately in the lower abdomen on both sides. 20% of patients with ovulation abdominal pain may also experience ovulation bleeding at the same time, but the amount of bleeding is not much and the duration is short, and it will heal without medical treatment within 2 to 3 days. Ovulation abdominal pain is a physiological phenomenon and generally does not require treatment. If the abdominal pain is severe and unbearable, you can take short-acting contraceptives orally to suppress ovulation for 2 to 3 cycles, which will help the abdominal pain disappear.

Corpus luteum rupture

After ovulation, a corpus luteum is formed, containing a small amount of fluid. If there is excessive bleeding in the corpus luteum and it ruptures, severe lower abdominal or one-sided abdominal pain will occur. If the amount of bleeding is heavy, the pain may spread to the entire lower abdomen and cause discomfort in the abdomen, abdominal pain that is resistant to pressure, and signs of shock. If there is no fever and vaginal bleeding, the bleeding from corpus luteum rupture may stop on its own. If the amount of bleeding is heavy, surgical treatment should be performed to suture the ovarian rupture.

Endometriosis

The endometrium should grow in the uterine cavity. If it grows outside the uterine cavity (except the myometrium), it is endometriosis. Patients may experience symptoms such as dysmenorrhea, menstrual disorders, infertility, fever, dyspareunia and acute abdominal pain. The infertility rate of this disease is 20% to 66%.

Abortion There are two types of abortion: early abortion and late abortion. A miscarriage that occurs before 12 weeks of pregnancy is called an early miscarriage; a miscarriage that occurs between 12 and 28 weeks of pregnancy is called a late miscarriage. The main symptoms of miscarriage are bleeding and abdominal pain. In early miscarriage, there is vaginal bleeding throughout the entire process, while in late miscarriage, the amount of bleeding is not much.

Ovarian cyst pedicle torsion

The ovary is a common site for tumors, and pedicle torsion occurs in approximately 10% of ovarian tumors. Most of these tumors are benign. They have long pedicles, large tumors, no adhesions to surrounding tissues, and high mobility, making them easy to twist. This is especially true for cystic teratomas, because these tumors have a long pedicle and their center of gravity is biased to one side, and the tumor is easily twisted by intestinal peristalsis or changes in body position.

Ectopic pregnancy

It refers to the process in which the fertilized egg develops into a blastocyst and implants and develops outside the uterine cavity, so it is also called ectopic pregnancy. Among them, tubal pregnancy is the most common. After becoming ill, 90% of patients seek medical attention due to sudden abdominal pain. At first, there is a tearing pain in the lower abdomen on the affected side, and then the pain spreads to the whole abdomen. The degree and nature of pain are related to the amount and rate of internal bleeding. If the fallopian tube ruptures and the internal bleeding is rapid and heavy, it will irritate the peritoneum and cause severe pain that will spread to the entire abdomen. In severe cases, symptoms of shock such as dizziness, blurred vision, cold sweats, palpitations, and even fainting may also occur. Patients with ectopic pregnancy may have a history of amenorrhea and irregular vaginal bleeding in addition to abdominal pain.
