The main manifestation is that weight does not increase, but decreases until emaciation. The order of subcutaneous fat reduction is abdomen, chest, back, waist, upper limbs, lower limbs, buttocks, and finally cheeks. Initially, patients are often irritable and have trouble sleeping, followed by apathy, or alternating depression and irritability, loss of appetite, often accompanied by vomiting or diarrhea, and some may be constipated. Body temperature is lower than normal, pulse is slow, basal metabolism is reduced, accompanied by various symptoms of vitamin deficiency, susceptibility to various infections, and varying degrees of functional disorders in various organ systems.

Clinically, malnutrition is divided into three degrees according to the severity of symptoms:

The first degree is that the body weight is reduced by 15% to 25% compared to the normal average body weight, and the thickness of the subcutaneous fat in the abdomen is between 0.8 and 0.4 cm;

In the second degree, the body weight is reduced by 25% to 40% compared with the normal average weight. The subcutaneous fat layer in the abdomen almost disappears, the limbs and buttocks are obviously thin, the facial fat has decreased, but there are no symptoms in the respiratory and circulatory organs.

In Grade III, the body weight is reduced by more than 40% compared to the normal average weight, and the subcutaneous fat layer on the face and buttocks has disappeared. The skin is pale, dry and has completely lost its elasticity. The body temperature is lower than normal, the heart sounds are dull, the blood pressure is low, and the breathing is shallow.
