1. Traditional Chinese Medicine

According to TCM diagnosis, amenorrhea can be divided into five types: deficiency of both Qi and blood, qi stagnation and blood stasis, yin deficiency and blood deficiency, stagnation of cold and dampness, and phlegm blocking Chong and Ren meridians.

(1) Qi and blood deficiency type: Menstruation often starts with a small amount and then stops. The patient is tired and short of breath, has a pale complexion, eats little and has loose stools, has a thin white tongue coating, and has a weak pulse. Treatment should be to replenish qi and blood. The prescription is Shiquan Dabu Decoction (ginseng, white atractylodes, poria, licorice, rehmannia, angelica, white peony, chuanxiong, huangmao, cinnamon) with modifications.

(2) Qi stagnation and blood stasis type: Symptoms include dark purple complexion, distension and pain in the chest and flanks, heaviness in the lower abdomen, purple spots on the tongue, and a deep and wiry pulse. The treatment should be based on regulating Qi, promoting blood circulation and removing blood stasis. The prescription uses the Lower Abdomen Expelling Stasis Field (Angelica, Chuanxiong, Cinnamon, Corydalis, Notopterygium, Pollen, Fennel, Paojiang, Myrrh), and add Qi-regulating and blood-activating drugs such as costusroot, Cyperus rotundus, peach kernel, and safflower as appropriate.

(3) Yin deficiency and blood deficiency type: Symptoms include fever in the five parts of the body, flushed cheeks, restlessness and insomnia, dry lips and tongue, dizziness and tinnitus, sore waist and weak knees, and a thready and weak pulse. The treatment should be to nourish yin and blood. The prescription is Daying Decoction (rehmannia, angelica, licorice, eucommia, Achyranthes bidentata, cinnamon, tung oil cake) with the addition of yin-nourishing and fever-reducing drugs such as Ligustrum lucidum, white peony root, Artemisia annua, and turtle shell.

(4) Cold-dampness stagnation type: Symptoms include bluish face and cold abdomen, chest tightness and distension, abdominal pain during bowel movements, cold and damp leucorrhea, white and moist tongue coating, and a deep and slow pulse. Treatment should be to warm the meridians and dispel cold, and to remove dampness and blood stasis. The prescription for warming the meridians is Erchen Decoction (Guizhou twig, Niuxi, Huoshu, Angelica, Chuanxiong, Moutan bark, White peony root, Ginseng, Licorice, Citrus aurantium, Shida chuanxiong, and Pinellia ternata), with modifications.

(5) Phlegm blocking Chong and Ren meridians: Symptoms include obesity, fatigue, shortness of breath, chest tightness and frequent sleepiness, a greasy white tongue coating, and a thin and smooth pulse. To treat directly, eliminate phlegm, dredge the meridians, and regulate the Chong and Ren meridians, you can use boiled Pinellia, Rhizoma Rhizoma Smilacis Glabrae, Rhizoma Crotons, Scabra, Peach Kernel, Safflower, Angelica, Cyperus rotundus, and Tangerine Peel to treat.
