Patients generally also have hematuria, oliguria and polyuria as the main manifestations, usually microscopic hematuria. Hematuria is aggravated by high incidence of infection, stones or post-traumatic hematuria. It is common in some patients with primary hydronephrosis. The lump may suddenly become smaller and the pain may be relieved after urinating a lot. When urination decreases, the lump may quickly expand and the pain may increase.

Lumbar and abdominal lumps: In the early stages of hydronephrosis, noticeable lumps will appear in the abdomen. The lump initially originates from below the rib margin, gradually widens toward the lateral abdomen and waist, and is nearly a cystic lump with a smooth surface that can be turned over the midline, with regular edges, a sense of fluctuation, and no obvious tenderness.

Spontaneous renal rupture: If the patient suffers a severe blow or in a non-invasive condition, hydronephrosis is very likely to cause secondary infection and renal pelvic rupture, leading to perirenal abscess and urine extravasation. The main symptoms are sudden pain in the waist and abdomen, with significant tenderness and muscle tension.

Hypertension: Hypertension is a common complication of hydronephrosis because long-term hypokalemia can easily lead to hypertension. About one-third of patients with moderate to severe hydronephrosis develop hypertension, which is mild or mild to moderate. It may be caused by the compression of the interlobular artery by the enlarged renal pelvis and calyces, resulting in renal parenchymal ischemia.
