Uterine incision diverticulum actually refers to poor wound healing, which is related to cesarean section. When a pregnant woman undergoes a caesarean section, it is easy for a wound to be left behind. This incision will form a depression that connects to the uterine cavity, and the depression is the diverticulum. For the occurrence of uterine incisional diverticulum, all patients need to actively deal with and treat it, because it poses a great threat to the patient's physical health.

The leading cause of uterine diverticulum is cesarean section. Many mothers choose cesarean section for their first child. There will naturally be an incision during surgery. When the uterine incision after cesarean section is not healed properly, a depression that communicates with the uterine cavity will appear at the incision. This depression is what we call a diverticulum. Because it occurs after a cesarean section, it is called a post-cesarean section uterine incision diverticulum.

Uterine incision diverticula are mostly located in the lower segment or isthmus of the uterus, and a few are located in the upper segment of the cervix. They are mainly related to the original cesarean section incision site and whether the cervix is ​​clinically shortened or expanded during cesarean section. The possible causes are:

1. The cesarean section incision is too high, and the structures of the cervical and uterine muscle tissues are different, which makes it easy for the incision to be misaligned. In addition, the incision is close to or on the cervix, which has less blood supply.

2. Infectious factors affect the normal repair of the cesarean section incision and form a diverticulum.

3. When there are foreign objects remaining in the uterine cavity, the discharge of the contents of the uterine cavity is blocked, the intrauterine pressure increases, and the weak part of the incision slowly bulges outward to form a diverticulum. In addition, multiple cesarean sections will increase the probability of diverticula.

In addition to causing difficulty in conception, uterine incision diverticulum may also cause the following symptoms: irregular vaginal bleeding, prolonged menstruation accompanied by dull pain and discomfort in the lower abdomen, abdominal pain during menstruation, pelvic mass, and heavy bleeding from the cesarean section incision during subsequent pregnancy. These will seriously affect the quality of life of pregnant women and even endanger their lives, so timely medical treatment is required.

Currently, the clinical diagnosis of uterine incisional diverticulum mainly relies on the above manifestations as well as vaginal ultrasound and hysteroscopy. The diagnosis can be confirmed when vaginal ultrasound shows one or more wedge-shaped or cystic fluid dark areas at the incision of the anterior wall of the lower uterine segment, which are connected to the uterine cavity, and the thickness of the uterine muscle layer is reduced; or when under hysteroscopy, the fibrous tissue at the lower edge of the incision of the anterior wall of the lower uterine segment forms a "valve" and old blood is seen in the depression.

Treatment of uterine incisional diverticulum after cesarean section includes hormone therapy, vaginal surgery, hysteroscopic surgery, open abdomen or laparoscopic surgery. Patients with recurrent or severe symptoms should undergo surgery actively. Surgery can remove the scar at the diverticulum site and re-suture the muscle wall, thereby removing the diverticulum and improving clinical symptoms.
