There are many small bumps on the nipples, which may be caused by breast eczema or sebaceous cysts, so female friends should pay attention to breast hygiene. During breastfeeding, the secretion of milk will cause the nipples to become moist, which is more prone to bacterial infection. Everyone should pay attention to the protection of the nipples.

1 Breast eczema

If there are small bumps near the nipples that are itchy and painful, it is very likely breast eczema. The causes of breast eczema are quite complex, including various external factors such as living environment, climatic conditions, etc., as well as some internal factors such as sexual digestive system diseases, gastrointestinal dysfunction, and mental stress.

Solution:

1. Western medicine treatment: Antihistamines can be used to relieve itching. In case of acute or subacute generalized eczema, 5% calcium bromide, 10% calcium gluconate or 10% sodium thiosulfate solution can be injected intravenously once a day, with 10 times as a course of treatment.

2. Traditional Chinese Medicine Treatment

The main treatment for acute eczema is to clear away heat and dampness. The prescription suitable is Longdan Xiegan Decoction or Bixie Shenshi Decoction combined with Ermiao Powder with modifications. If there is a purulent infection, you can add Viola yedoensis, dandelion, honeysuckle, forsythia, etc.

2 Sebaceous cyst

If there are small bumps near the nipples that are itchy and painful, it may be that sebaceous cysts have grown around the breasts. The name sebaceous cyst may sound unfamiliar, but it is actually what we call cysts in our daily lives. It is mainly caused by obstruction of the sebaceous gland excretion ducts, and the sebaceous gland cystic epithelium is expanded by the gradually increasing contents to form a retention cyst.

Solution:

1. Surgical removal of sebaceous cysts is a small tumor on the body surface. The operation is simple and can be performed in an outpatient clinic. The cyst should be removed as completely as possible without leaving any cyst wall, otherwise it is prone to recurrence.

2. CO2 laser and electric ion minimally invasive methods are considered to be good methods for treating cysts without concurrent infection. Due to their simple operation, small incision, little bleeding, no need for sutures, almost no scars, and low recurrence rate, they are particularly suitable for the treatment of sebaceous cysts on the face.

3. Nipple inflammation or areola inflammation

Small itchy bumps and pain near the nipples may also be caused by papillitis or areolar inflammation, which is mainly a purulent inflammation of the nipple and areola skin and glands caused by infections such as Staphylococcus aureus. It often occurs during the lactation period of women. If not handled properly, it can develop into mastitis. Traditional Chinese medicine calls this disease "nipple wind".

Solution:

1. Antibiotic treatment can be administered orally or intramuscularly. Such as cephalexin 0.25g, 4 times/d, orally; or cefadroxil (cefotaxime) 0.25g, 4 times/d, orally; or lincomycin (lincomycin) 0.25g, 3 times/d, orally; erythromycin 0.5g can be used orally, 2 times/d.

2. Treatment with Traditional Chinese Medicine Traditional Chinese medicine believes that this disease is related to "liver qi stagnation and internal heat stagnation", and the treatment principle should be "soothing the liver, promoting qi, clearing heat and detoxifying".

3. Surgical treatment: For those who have formed subareolar cellulitis or abscess, incision and drainage should be performed.

4 Mastitis

Mastitis may also cause small bumps to grow near the nipples and cause itchy and painful symptoms. Mastitis can be divided into acute suppurative mastitis, paraareolar fistula, plasma cell mastitis, etc. according to the different causes.

Solution:

1. Antibiotics: Antibiotics are used systemically. To prevent and treat severe infection and sepsis, antibiotics are selected based on bacterial culture and drug sensitivity, and intravenous antibiotics are given when necessary.

2. Block the breast with 60-80 ml of 0.25% procaine to reduce inflammation. Choose broad-spectrum antibiotics orally or intravenously. One million units of penicillin can be dissolved in 20 ml of normal saline and injected around the inflammatory mass.

3. If an abscess has formed, it should be drained by incision. The incision should be radial to the nipple, avoiding the areola. For retromammary abscess or deep abscess under the breast, an arc-shaped incision can be made at the sternotomy fold under the breast.

5. Breast hyperplasia

Breast hyperplasia is a relatively common breast disease, which can also cause small bumps and itching and pain near the nipples. The cause of breast hyperplasia is mostly due to endocrine disorders.

Solution:

1. Western medicine treatment can use hormone drugs, iodine preparations and tamoxifen, which can relieve pain. However, due to certain side effects, it is not the first choice. Vitamins A, B6, and E also have the function of regulating sex hormones and can be used as auxiliary medications for breast hyperplasia.

2. Surgical treatment of fibrocystic breast disease is caused by endocrine metabolic imbalance and there is no surgical indication in itself. In clinical practice, if there are individual breast nodules that are difficult to distinguish from breast cancer, surgical resection can also be used and the diagnosis can be confirmed by pathological examination.

3. Treatment with traditional Chinese medicine. Traditional Chinese medicine believes that breast hyperplasia begins with liver depression, and then blood stasis and phlegm coagulate into lumps. The treatment should be to soothe the liver and regulate qi, promote blood circulation and remove blood stasis, soften and disperse nodules. Bupleurum, white peony, Cyperus rotundus, citrus leaf, Salvia miltiorrhiza, and earthworms are commonly used medicines in traditional Chinese medicine prescriptions.
