What is vaginal bleeding during 37 weeks of pregnancy?

1. Placental detachment

Normally, the placenta separates from the uterine wall and is delivered only after the fetus is delivered. If the placenta partially or completely separates from the uterine wall before the fetus is delivered, it is called placental abruption. The cause of early placental abruption is not very clear. It may be related to factors such as pregnancy-induced hypertension, high blood pressure, trauma (such as abdominal impact, sexual intercourse, falls, heavy physical labor and severe coughing), sudden decrease in intrauterine pressure, sudden reduction in uterine cavity volume, and lying on the back for too long. Early placental abruption is more dangerous to pregnant women and fetuses than placenta previa, because the placenta has been partially or completely separated from the uterine wall before the fetus is delivered, but the uterus cannot contract and therefore cannot stop bleeding. If the separation starts from the edge of the placenta, blood may flow out along the uterine wall and the amniotic membrane to the cervix. This is called external bleeding type and the condition is relatively mild. If the abruption starts from the center of the placenta, the blood will stay between the placenta and the uterine wall and will not be able to flow out. As blood accumulates more and more, the uterus swells. The blood may seep into the myometrium, causing the uterine muscle fibers to separate and break, resulting in the loss of contraction ability. This is called internal bleeding. In addition, there are patients in which part of the blood remains in the uterine cavity and part flows out of the vagina. This is called a mixed type.

2. Symptoms

In addition to vaginal bleeding, most pregnant women with early placental abruption have persistent abdominal pain, enlarged and hardened uterus (some are as hard as a board), tenderness, unclear fetal heart rate, and in severe cases, shock. If rescue is not timely, it may cause death. Therefore, if abdominal pain and/or vaginal bleeding occurs in late pregnancy, you should go to the hospital for diagnosis and treatment as soon as possible. If placental abruption is suspected, you should be hospitalized for treatment. It is generally required to complete delivery within 6 hours of onset to ensure the safety of mother and baby. If the uterus is found to have lost its ability to contract and bleeding does not stop, the uterus should be removed.

The blood sinuses at the edge of the placenta are ruptured, the amount of bleeding is not much, and the threat to the fetus is relatively small, but you still need to seek medical attention in time and let the doctor make the judgment. Signs of premature birth. If vaginal bleeding occurs accompanied by regular uterine contractions, it may be a sign of premature birth and you should go to the hospital immediately.
