How should plasma cell mastitis be treated?

Plasma cell mastitis is a chronic non-bacterial inflammation with complex causes, including nipple inversion, nipple splitting and breast dysplasia, and it often occurs in middle-aged and elderly women. Once affected, the nipples usually produce a foul-smelling discharge that may be red in color. So, how is plasma cell mastitis treated?

Plasma cell mastitis is a chronic nonbacterial inflammation of the breast. Due to the complex and changeable pathological characteristics, there are many names for it, such as "occlusive mastitis", "lactation mastitis", "chronic mastitis", "mammary duct ectasia", "acne-like mastitis", etc. Plasma cell mastitis mostly occurs in middle-aged and elderly women, with the peak age being 50 to 60 years old. Most patients have inverted nipples. Plasma cell mastitis is different from general lactation suppurative mastitis. Many people do not recognize this disease and mistake it for a general bacterial infection or breast tuberculosis. The most terrible thing is that it is misdiagnosed as breast cancer and the breast is removed by mistake.

Clinical characteristics

1. It is not related to pregnancy or breastfeeding, that is, the disease does not occur during the breastfeeding period.

2. Most patients have various nipple deformities or duct dilatation.

3. There are many young women, and quite a few of them are unmarried.

4. Recurrent and long-lasting paraareolar fistula or chronic inflammatory mass. In one case, the history of the disease lasted for 13 years.

5. This disease is not uncommon, accounting for about 10% of breast patients.

Treatment

1. Anti-inflammatory treatment in the acute phase. Since it is not caused by bacteria, there is no need to use antibiotics. Traditional Chinese medicine can clear away heat, detoxify, reduce swelling and resolve nodules. However, it is not advisable to use too much bitter and cold medicine. The more cold medicine you use, the slower the lump will go away.

(1) Oral hormone therapy with dexamethasone or prednisone, gradually reducing the dosage, combined with oral anti-inflammatory drugs for anti-inflammatory treatment.

(2) At the same time, take Chinese medicine orally to warm the yang, resolve phlegm, reduce swelling and disperse nodules.

2. In the chronic stage, use warm and hot medicine - Yanghe Decoction with modifications.

3. Choosing the best time for surgery is the most important.

The interictal period, that is, the wound healing period, is the best time for surgery, but some people think that why do they need surgery after the disease has healed? They wait until the disease becomes red, swollen, and ulcerated again, which delays it for a long time.

(1) During an attack, Chinese medicine can be applied externally to relieve pain if necessary.

(2) At the same time, cephalosporins should be given intravenously for anti-inflammatory treatment. During the ulceration period, dressings should be changed actively, or hanging thread therapy should be adopted.

If the wound cannot heal, surgery should be performed when the acute inflammation subsides and the wound is at its most superficial. At this time, there is a possibility of infection after the surgery.

4. The key to a successful operation is to flip the areola, completely remove the lesions, and clean all wound surfaces.

5. The technical key to the operation is to maintain the perfect appearance, and nipple inversion surgery is necessary.
