How to treat anovulatory functional uterine bleeding? Choose according to age

Anovulatory functional uterine bleeding is more common in female adolescence and menopause. The treatment of anovulatory functional uterine bleeding should choose the appropriate method according to the patient's age. If the patient is an adolescent, the key principles should be to stop bleeding and promote ovulation, while in menopause, the key principles should be to stop bleeding and reduce menstruation.

(1) General treatment

Improve general condition, correct anemia, pay attention to rest, and strengthen nutrition.

(2) Curettage and dilatation

Especially for menopausal patients with recurrent bleeding, segmented diagnostic curettage can be performed, which can quickly stop bleeding and provide a clear diagnosis.

(3) Hormone therapy

① For adolescents with anovulatory dysfunctional uterine bleeding, 2 mg of estradiol benzoate can be used, injected intramuscularly once every 6 to 8 hours. After the blood volume decreases significantly, the dosage can be gradually reduced. The dosage should not exceed 1/3 of the dosage each time. When it is reduced to 1 mg, it can be taken orally instead. The cumulative medication is used for a total of 20 days. In the next 3 days, 20 mg of progesterone can be added intramuscularly every day.

② For those who have no ovulation and want to have children, take sucralose orally starting from the 5th day of menstruation, 50 mg each time, once a day, for 5 consecutive days.

③ For menopausal women with anovulatory dysfunctional uterine bleeding, norethindrone (Fukang tablets) can be used, 5-8 mg each time, once every 6 hours. When the blood volume decreases or stops, the dose is reduced by 1/3 every 3 days until the maintenance dose is 5 mg per day, and it is maintained for a total of 22 days.

(4) Treatment with traditional Chinese medicine (TCM), such as Dingkun Dan and Wuji Baifeng Wan.

(5) Surgical treatment

If conservative treatment is ineffective, bleeding is excessive, and severe anemia occurs, hysterectomy may be considered depending on age.
